Evidence of Ongoing Immune Reconstitution in Subjects with Sustained Viral Suppression following 6 Years of Lopinavir-Ritonavir Treatment

Abstract: The receipt of a lopinavir-ritonavir-based regimen resulted in ongoing immune reconstitution through 6 years of therapy in a cohort of HIV-1-infected, antiretroviral-naive subjects with suppressed HIV-1 RNA levels. Normalization of activation marker expression on CD4+ and CD8+ T cell subsets was demonstrated.

“…1A. As reported by others (21,22), both CD4 and CD8 cells showed increased CD38/ HLA-DR expression in ART 2 patients (p = 0.0007 and p , 0.0001, respectively), which was significantly decreased in ART + patients (p = 0.01 and p = 0.0005, respectively) (Fig. 1B).…”

“…We have previously described patients with HIV infection who received an effective lopinavir and low-dose ritonavirbased ART regimen for 6 years, after which time, the proportions of activated (CD38 + , HLA-DR + ) CD4 + and CD8 + T cells were significantly lower than they were in untreated patients with HIV infection and comparable to what they were in HIVnegative subjects [8]. It is unknown what effect receiving ART for this length of time has on levels of serum immune activation markers or what relationship exists between serum immune activation markers and CD4 + T cell counts and T cell activation.…”

“…Serum levels of IgG, IgA, and IgM were assayed by immunoturbidimetry (Abbott Architect 8200 Chemical Analyzer). Total and activated CD4 + and CD8 + T cells were enumerated by flow cytometry, as described elsewhere [8].…”

“…Lopinavir-ritonavir (LPV-r) is a PI combination well known for its potency and efficacy. In antiretroviral-naive adults, LPV-r demonstrates sustained virological suppression for up to 6 years in 195% of patients [3]. LPV-r use in children in resource-limited settings has also achieved good outcomes over sustained periods as long as 4 years [4].…”
