Evidence-Based Physiotherapy for Acute Low Back Pain: A Composite Clinical Algorithm Synthesized from Seven Recent Clinical Guidelines

Abstract: Purpose: To produce a composite evidence-based treatment algorithm for physiotherapy management of acute low back pain (LBP) using current, high-quality, English-language clinical guidelines. Methods: A systematic literature review of library databases and Internet search engines was performed to identify full-text, Englishlanguage clinical guidelines on the physiotherapy treatment of acute LBP. Quality assessment of the guidelines was undertaken by two independent reviewers using the AGREE instrument. Guidel… Show more

“…Well-constructed guidelines which follow a documented and agreed process aim to provide believable syntheses of current research evidence. 51 This supports operationalization of evidencebased practice by "average" practitioners for "average" patients using clinical reasoning relevant to discipline training, patient preference, and local environments. High quality guidelines are regularly produced by credible agencies such as the SIGN, 52 NICE, 53 NHMRC, 54 and NZGG.…”

“…55 These guidelines offer a ready way for AH practitioners to access carefully evaluated and synthesized information on best practice care. 51 Under the medical model, evidence ranges from acceptable clinical opinion through to scientifically rigorous published evidence. Where scientific published evidence is available, it is generally empirical.…”

“…This approach is less commonly applied in critical care, but it can be a pragmatic method to support clinical decision making, particularly in areas where research is lacking. In addition, the formulation of evidence-based clinical guidelines and/or best practice recommendations (BPR) has been proposed as a means of facilitating clinical decision making [21,22,[27][28][29][30][31][32]. While algorithms developed by a group of recognized experts who appraise and contextualize evidence in the field, may facilitate the translation of BPR into clinical practice [21,30,33,34].…”

While algorithms developed by a group of recognized experts who appraise and contextualize evidence in the field, may facilitate the translation of BPR into clinical practice [21,30,33,34]. The cost effectiveness of using practice guidelines lends further support for developing an evidence-based clinical management algorithm (EBCMA) for the management of PDF in this expensive setting [35][36][37].…”

“…The key words entered into http://www.google.com were "clinical guidelines" and "acute low back pain". The article published by Holohan et al (2005) was used to trace some of the clinical guidelines (Holohan et al 2005). Only clinical guidelines published on acute LBP were included.…”
