Conflictos de interés: los autores declaran no tener conflictos de interés. RESUMENIntroducción: La hernia incisional gigante con saco ocupado por un útero grávido es una entidad rara, pero representa una condición ginecológica y obstétrica con potenciales complicaciones tanto para la vida de la madre como para la del hijo.Caso clínico: Presentamos a una paciente de 32 años con embarazo de 26.4 semanas, con útero grávido en el saco de una hernia incisional poscesárea, úlceras y escaras en la piel de la pared abdominal. Se asocia como morbilidad una obesidad con índice de masa corporal de 37.5 kg/m 2 y una diabetes gestacional. Se realiza un manejo personalizado y multidisciplinario que le permite llegar a las 35 semanas de gestación y realizar cesárea electiva, hernioplastia incisional anatómica y dermolipectomía de necesidad, con lo que se logra un resultado materno y fetal satisfactorio.Conclusión: Se considera que las pacientes embarazadas que presentan el útero gestante en el saco herniario tienen un embarazo de alto riesgo. Durante su manejo es recomendable realizar una valoración y una conducta personalizadas por un equipo multidisciplinario que efectúe, tanto como sea posible, una planificación táctica segura, con una actitud terapéutica conservadora para lograr un final del embarazo satisfactorio y un adecuado cierre parietal.
