Objective: Sarcoidosis is a chronic systemic inflammatory disease which can affect multiple organ systems. The role of biomarkers in the diagnosis and prognosis of sarcoidosis is increasing. Interest in the role of adipose tissue mediated inflammation in the pathogenesis of the inflammatory diseases has increased in recent years. Visfatin is a proinflammatory adipocytokine which has been studied for several inflammatory diseases such as diabetes mellitus, obesity and metabolic syndrome. Our aim is to assess serum visfatin levels in sarcoidosis and its relation with other markers of inflammation (CRP, ACE and ESR). Material and Methods: We enrolled 59 patients with sarcoidosis and 21 healthy controls. We measured plasma levels of visfatin along with serum CRP, ESR, ACE using ELISA (enzyme-linked immunosorbent assay) kits (Blue Gene Biotech, Shanghai, China). Results: Visfatin levels were not significantly different between patients and control subjects (29.9±15.8 ng/ml for patients and 23.93±16.73 ng/ml for controls, p=0.15) and there was no correlation between visfatin and serum CRP, ACE or ESR in patients with sarcoidosis. Conclusion: Visfatin recently is being discussed as biomarker for inflammatory diseases in several studies and results are controversial. In our study, no differences were found in serum level of visfatin between patients with sarcoidosis and control group.

Lupus nephritis (LN) affects up to 60% of patients with systemic lupus erythematosus (SLE), either as the initial manifestation or during the disease course. Moreover, LN has a negative impact on survival of SLE patients. Accordingly, it is mandatory to identify specific and feasible markers able to guide clinicians towards the adequate therapeutic option in LN patients. The aim of this work was to evaluate the predictive value of renal resistance index (RRI), measured by Doppler Sonography in comparison with disease activity score, serologic and biopsy parameters in patients with LN. This study was carried out on forty three SLE patients, they were categorized into two groups: Group I included thirty three patients with LN and Group II included ten patients without LN and Group III included ten healthy subjects of matched age and sex as control group. All were subjected to history taking, clinical examination, assessment of disease activity by SLEDAI, laboratory investigations including FBG, blood urea, serum creatinine, serum albumin, CBC, ESR, CRP, complete urine analysis, 24 hour urine protein, eGFR, serum ANA, anti ds-DNA titre, C3, C4 and renal Doppler with measurement of RRI. Renal biopsy was done for those with LN. The mean value of RRI was statistically significantly higher in group I than that of group II and group III. Out of 33 cases of LN cases, 6 patients had RRI of 0.7 and above giving a percentage of 18.18%. LN patients with RRI higher than 0.7 had statistically significant higher age, mean serum creatinine and blood urea levels and a lower eGFR, higher chronicity index of renal biopsy. RRI is of clinical significance in predicting the chronicity index of renal biopsy which is a major determinant of renal outcome so it is useful as non invasive technique to evaluate chronicity in patients with LN, therefore justifying aggressive immune suppression but further follow-up studies are needed to evaluate its role in predicting response to treatment.

Introduction: Lupus nephritis (LN) is common and carries a high burden of morbidity in SLE patients. The adipokine ''visfatin'' is highly expressed in visceral fat, exerts several pro inflammatory functions and was demonstrated as a marker of endothelial dysfunction (ED) in chronic kidney disease (CKD). Aim of the work: To evaluate the state of serum visfatin in SLE patients and to detect its possible correlation to disease activity and kidney affection. Also, to define a possible correlation between the level of visfatin before and after a treatment regimen of combined mycophenolate mofetil and corticosteroids. Patients and Methods: visfatin was assayed using enzyme-linked immunosorbent assay (ELISA), chemical and immunological markers of SLE and LN were measured in 50 SLE patients (included 25 patients with LN and 25 patients without LN), and compared with 25 age and sex matched healthy controls. Disease activity was assessed using SLE Disease Activity Index (SLEDAI). Renal biopsies were taken from LN subgroup and were classified according to the modified WHO classification. 1 Results: There was statistically highly significant difference (P<0.001) as regards serum visfatin between patients (mean 8.31±3.60, median 8, 64), and controls (mean 4.60±2.01), serum visfatin showed significantly higher levels in LN compared to the non-lupus nephritis group (mean 8.78±3.81 ng/ml,) versus (mean 7.85±3.38 ng/ml,). Also, visfatin level was significantly higher among the active (mean 9.30±3.14,) compared to the inactive group (mean 4.37±2.46). Visfatin had a highly significant positive correlation with SLEDAI, disease duration, corticosteroids treatment duration, ESR and CRP. Also, a significant inverse correlation existed between visfatin, WBCs and Platelets count, correlation studies between visfatin level and low level of C3, C4 were significant. The correlation between serum visfatin level and Carotid artery intima media thickness by carotid Doppler imaging was also significant. There was a significant decrease (P=0.041) between pretreatment visfatin level and post treatment level after 3 months of MMF and high dose CS treatment, Visfatin mean decreased from (8.78±3.81 ng/ml) to (8.29±3.44 ng/ml). Conclusion: visfatin is closely associated with SLE activity especially with lupus nephritis revealing the promising role of this adipokine in SLE activity measurement and prediction of renal involvement in SLE patients. Also, treatment regimen of combined mycophenolate mofetil (MMF) and corticosteroids is effective in decreasing visfatin level in Lupus nephritis patients as well as SLE activity, thus it helps in achieving disease remission.
