Evaluation of the Quality of Adjuvant Endocrine Therapy Delivery for Breast Cancer Care in the United States

Abstract: From 2004 to 2013, underuse and misuse of AET have decreased for patients with breast cancer, but optimal use has not been achieved, and significant variation in care remains. The involvement of surgery and radiotherapy were among the most significant factors associated with optimal use, which underscores the benefits of team-based care to support guideline-concordant therapy.

“…Tamoxifen is a treatment option for both pre‐menopausal and post‐menopausal women (Early Breast Cancer Trialists’ Collaborative Group, ). Despite the prognostic benefits of ET, the adherence to treatment varies, and 30%–70% of the patients discontinue their treatment within 5 years (Daly et al, ; Tinari et al, ; Ursem et al, ). Non‐adherence mainly occurs during their first year of treatment (He et al, ), partly because ET is associated with several adverse side effects, such as hot flashes, depression, nausea and oedema (Regan, Price, Giobbie‐Hurder, Thürlimann, & Gelber, ).…”

“…However, the adherence and persistence to hormonal treatment has been shown repeatedly to be low in clinical practice, with < 50–70% of HR positive disease patients receiving adjuvant hormonal therapy during the period our patients were evaluated [32]. Even more recent publications continue to show the same pattern of lower adherence to adjuvant hormonal therapy, especially in patients < 40 yo [33]. Current guidelines advocate hormone therapy for all young patients with HR positive tumors [34].…”

“…Several of the highest priority research gaps centered on improving delivery of care, including interventional studies to improve adherence to medication. This ranked highly in part because it is common, negatively impacts outcome, and in particular affects vulnerable breast cancer patients such as African-American (3638) and women at both ends of the age spectrum. (36, 3942).…”

(36, 3942). Comparative effectiveness, namely the comparison of existing approaches to determine greatest benefits and harms, was also felt to be an understudied topic, and one ripe for the cooperative group setting since these are difficult to fund studies outside of the federal sphere, and often have substantial public policy implications.…”
