We tested the validity of an new methodological approach to the calculation of oxygen uptake efficiency slope (OUES) [i.e. the use of exercise end-points based on fractions of heart rate reserve (HR(res))], as an alternative to the traditional time-based calculation. Twenty-nine healthy sedentaries >60 years of age (18 males, 11 females) performed an incremental cycling exercise to exhaustion. Respiratory variables and HR were measured breath by breath. Resting and peak variables were calculated and ventilatory threshold (VT) was identified by standard technique. OUES was calculated on 75, 90 and 100% of the incremental exercise data (OUES75, OUES90, OUES100) and on data corresponding to 60 and 80% of the HR(res) (OUES60%HR(res), OUES80%HR(res)). No significant difference (repeated measures ANOVA) was detected between time-based (OUES100, OUES90, OUES75) as well as HR(res)-based measures of OUES (OUES80%HR(res), OUES60%HR(res)). The Bland-Altman analysis revealed a bias not significantly different from 0 (22.0 and 53.3 for OUES80%HR(res)-OUES100 and OUES60%HR(res)-OUES100, respectively), a precision of 171.2 and 289.0 and 95% limits of agreement from -313 to +358 and from -513 to +620 for OUES80%HR(res)-OUES100 and for OUES60%HR(res)-OUES100, respectively. High correlations were detected between (VO(2peak)) and OUES60%(res) and OUES80%HR(res) (r (2) = 0.70 and 0.81, respectively) and between VT and OUES60%(res) and OUES80%HR(res) (r (2) = 0.58 and 0.66, respectively). The main finding of this study is that OUES can be reliably calculated based on HR(res) end-points during incremental cycling exercise, in healthy elderly subjects. Furthermore, our study confirms the validity of OUES as an indicator of aerobic exercise capability in this population.

SummaryThe oxygen uptake efficiency slope is a measure of cardiopulmonary fitness, that can be obtained from a sub-maximal cardiopulmonary exercise test. It has been evaluated in medical patients but its reliability in surgical populations remains uncertain. We conducted a test-retest study with the aim of establishing the reliability of the oxygen uptake efficiency slope in relation to that of the anaerobic threshold and peak oxygen uptake, in general surgical patients. Twenty-six patients over 60 years old completed two symptom-limited, incremental cycle ergometry tests within 7 days. The mean (SD) anaerobic threshold was 13.0 (3.0) mlO 2 .kg )1 .min )1 . There were no significant differences between mean test and retest values of anaerobic threshold (p = 0.50), peak oxygen uptake (p = 0.76) or oxygen uptake efficiency slope (p = 0.42). Reliability coefficients (95% CI) for the anaerobic threshold, oxygen uptake efficiency slope and peak oxygen uptake were 66.7% (45.3-87.9%), 89.0% (81.0-96.9%) and 91.7% (85.7-97.8%), respectively. The oxygen uptake efficiency slope was determined easily in all patients and found to have excellent reliability. Its clinical utility in determining pre-operative fitness warrants further evaluation.

The development and organism change, especially changes in biological maturation process, is impressive on the physiological parameters in response to exercise. Hence, the new index oxygen uptake efficiency slope (OUES) role with biological maturity intervention and anthropometric factors of body weight, BSA and LBM which can measure the cardiorespiratory function in children maturity level normal and abnormal (early or late) with high health and safety is important. 72 healthy young males, with a mean age 13.95 ± 1.8 years and BMI 19.91 ± 3.4 kg/m 2 were separated into two groups of normal puberty (n = 33) and abnormal maturation (n = 39). After consent obtaining, their VO 2max was determined by maximal aerobic test via gas analyzer (VE, VO 2 and VCO 2 ). The new index OUES was also determined according to logarithmic relationship Baba. A high correlation was observed between VO 2max and OUES physiological index (R 2 = 0.90, SEE = 292). There was no significant difference in the index OUES with and without the intervention of weight, BSA and LBM between normal and abnormal maturation of boys (p > 0.05). Significant correlation between VO 2max and normalized OUES with anthropometric variables was obtained (R = 0.83, R = 0.87, R = 0.91, p = 0.00). OUES is influenced by weight, BSA and LBM. But the BSA role in the index of cardiac performance was more sensitive than the other two parameters. It appears that due to the lack of intervention maturation effect on this index, during the study of cardiorespiratory system physiological responses of children to exercise, regardless of the maturity the application of this index is possible. © 2015 Consell Català de l'Esport.
