Evaluation of the discrepancy between clinical diagnostic hypotheses and anatomopathological diagnoses resulting from autopsies

Abstract: OBJECTIVES:An objective of clinical autopsies is to determine the final cause of death and the pathological changes that may have triggered it. Despite advances in Medicine, the level of discrepancy between clinical and autopsy diagnoses remains significant. The aim of this study was to compare the data obtained from autopsies carried out at the São Bernardo do Campo/SP Death Verification Section with clinical diagnostic hypotheses proposed during medical care.METHOD:This was a retrospective study involving th… Show more

“…Gender and age of decedents did not play a role for the frequency of major discrepancies in our study. Previous studies come to different results when regarding the influence of gender and age on the rate of discrepancies [9,14,18,22,23]. When a difference in the frequency of discrepancies was detected, more discrepancies were found in older patients and in women [14,22,23].…”

“…In the subsequent almost 30 years, medical progress has accelerated considerably. In recent publications however, the rate of major discrepancies remains considerably high with 19.5-28% [3,[9][10][11]. In oncological patients, detailed studies of postmortem examinations are lacking.…”

“…The autopsy can provide significant insight into disease progression and treatment efficacy, and establishing an accurate cause of death ensures that reliable mortality statistics are available for public health planning. Despite advances in antemortem diagnostic techniques and postmortem imaging, clinicopathological discrepancies have remained high 1–4 . Certain pathological features are only visible microscopically and gross examination of organs is fallible: histopathology can improve the quality of autopsy reports and information available to families, clinicians, coroners and public health services 3,5–7 .…”

“…1 Previous studies have predominantly utilized autopsy data to determine discordance rates among pre-and post-mortem diagnoses and to identify pathologies at highest risk of misdiagnosis. Examples of such work that has provided valuable data to guide healthcare policy from around the world include pre-and post-mortem discordance rates of 9.3% in India, 2 12.3% in Canada, 3 17.2% in the USA, 4 18.1% in the Netherlands, 5 19.0% in Greece, 6 25.6% in Spain, 7 28% in Brazil, 8 and 48.4% in Jamaica. 9 Although this area of work has contributed considerably to the practice of diagnostic medicine, it is only beginning to scratch the surface of the potential applications of autopsy data.…”
