Minimally invasive percutaneous nephrolithotomy (mini-PCNL) and retrograde intrarenal surgery (RIRS) are both alternatives for PCNL to treat renal calculi. This study is aimed at comparing the stone-free rate (SFR) and other surgery parameters of two approaches for treating upper urinary calculi. We performed this meta-analysis in September 2016 by searching studies about mini-PCNL and RIRS for treating upper urinary calculi in various databases, and RevMan v.5.3 was applied. Three randomized controlled trials and ten nonrandomized trials were included, involving a total of 1317 patients. Meta-analysis showed that mini-PCNL group led to a higher SFR [odds ratio: 1.96; 95% confidence interval: 1.46–2.64; P < 0.00001] but brought a larger postoperative decrease in hemoglobin levels compared with RIRS. RIRS provided a shorter hospital time. There was no significant difference in operation time. Higher postoperative complications were detected in the mini-PCNL, but the difference was not significant. Grade I and III complications did not vary between two procedures, but grade II complications were of lower incidence in RIRS group. In the light of these results, compared with RIRS, mini-PCNL provided significantly higher SFR and efficiency quotient for managing calculi; however, it resulted in higher incidence of postoperative complications, larger hemoglobin drops, and longer hospital stay.

In this article, we aimed to compare the complications occurring after laparoscopic radical (LRN) and simple nephrectomy (LSN). Methods: After obtaining local ethics committee approval, we analyzed the records of a total of 98 patients, who underwent transperitoneal laparoscopic nephrectomy. LSN and LRN were performed in 64 and 34 patients, respectively. The Clavien-Dindo classification was used for evaluating complications. Pneumoperitoneum was created using a Veress needle. Intraabdominal carbon dioxide pressure was 18 mmHg while three 10 mm trocars were inserted and then one 5 mm trocar was inserted if needed. Results: The mean age of the LSN and LRN patients was 38.8±18.43 and 55.7±14.75 years, respectively. Five patients who underwent LSN developed postoperative minor complications (fever in one patient, nausea and vomiting in four patient). In addition, one patient who underwent LSN died postoperatively (vascular injury). Minor complications (fever in two patients, nausea and vomiting two, and ileus in one patient) developed postoperatively in five of 34 patients in LRN group. Conclusion: The rate of complication after LRN was found to be statistically significantly higher than that after LSN.

Objective: With the advent of laparoscopic approach for the large (T1b–T3a ± N1) right renal masses, higher rates of complications and conversion to open surgery are being reported. The role of preoperative angioembolization (PAE), which has increased cost and inherent morbidity but may help in select circumstances has also not been clearly defined in the literature. We therefore devised a scoring system (SGPGI score) based on pre-operative Computed Tomography Angiography (CTA) to predict the level of difficulty of radical nephrectomy and enhance its safety and efficacy which could also be used for the judicious use of PAE in selected cases. Methods: In a prospective observational study on 52 patients with right renal masses from January 2014 to July 2018, we calculated a score based on CTA parameters. The patients were stratified for type and duration of surgery, blood loss, postoperative stay, and Clavien-Dindo grade of postoperative complications. Results: Patients were classified into three groups based on our scoring system. Progressively groups with higher score had higher blood loss, operating time, complications and hospital stay, and were more likely to have undergone conversion to open surgery (Area under curve 0.8625 for a cut off score of 10). Intraclass Correlation Coefficient (ICC) was 0.678–1 for the different components of our score. Conclusion: The pre-operative CTA based SGPGI score evaluates right renal masses and is able to predict intra-operative difficulties effectively, leading to enhancement of surgery safety and efficacy. It also helps judiciously use PAE.
