Evaluation of the appropriateness of blood and blood product transfusion using preset criteria

Abstract: The use of 1930 units of blood or blood products during 765 transfusion episodes in 560 patients was reviewed. This represented one-half of all transfusions in a large medical center over a 2-month period. By clearly defined, present criteria, 42.3 percent of the episodes were found not to be appropriate. Whole blood and platelet transfusions were used more appropriately than those of red cells, cryoprecipitate, and fresh-frozen plasma. Blood transfusions were used more appropriately in the management of acute… Show more

“…These 2 episodes (25%) of inappropriate platelets transfusion were in neonatal intensive care unit. Emergency situations in intensive care settings may lead to inappropriate use of platelets and fresh frozen plasma too [8,13,15]. In the present study, 83.15 % of the transfusion episodes were appropriate.…”

“…[8] Training of physicians plays a major role in management of blood usage, since human errors are involved in the risks of blood transfusion therapy. [9] Studies carried out by Mozes et al [10] Surgenor et al [11] and Goodnough et al [12] since the onset of the Acquired Immunodeficiency Syndrome (AIDS) epidemic have found that there is still considerable inappropriate use of blood products. Correlation between physicians' knowledge, usage of blood components and the inappropriate use of blood products have been demonstrated in studies conducted by Salem-Schatz et al [13] Some studies conducted by Wu et al [14] Hebert and Fergusson.…”

“…This leads to improper use of blood and blood products, more costs per patient and the exposure of patients to infectious and noninfectious adverse consequences. [2,8,10,11,13,17] Significant advances in internal medicine and aggressive surgeries have led to remarkable increase in use of blood and blood components. However, many of the physicians are still unaware regarding the emergency blood transfusion protocols and massive blood transfusion protocols.…”
