Evaluation of sweating responses in patients with collagen disease using the quantitative sudomotor axon reflex test (QSART): a study protocol for an investigator-initiated, prospective, observational clinical study

Abstract: IntroductionSweat secretion is controlled by the sympathetic nervous system and is less active during winter than in the summer. Raynaud’s phenomenon is affected by an excessive strain of the sympathetic nerves after exposure to a cold environment, thus reducing the quality of life of patients with collagen disease. Herein, we focus on the eccrine sweat glands that receive both adrenergic and cholinergic innervation. Our hypothesis is that excessive activation of sympathetic nerve in Raynaud’s phenomenon can a… Show more

“…Quantitative sudomotor axon reflex test (QSART) is designed to stimulate the autonomic nervous system and evaluate how nerves that regulate sweat glands responds to stimulation. A SSc QSART protocol for skin symptoms (digital ulcers, pernio-like eruptions, subcutaneous calcifications, telangiectasia, nailfold capillary dilatation/bleeding and degree of skin sclerosis) and skin surface temperature is under investigation in Japan for a observational clinical study [59]. This study adopts two evaluation points, summer and winter, to observe effects of temperature on sweating.…”

“…We analyzed the association of the ARSV with the severity of Raynaud's activity, defined by the RCS. The total scores were dichotomized as low scores (0–7) and high scores 8–16 (Figure 3a). Values for the subcomponents of attack, pain, color, and duration were dichotomized into those without symptoms (0) and those with symptoms (≥1) (Figure 3b).…”

“…Autonomic dysfunction is an important factor of morbidity and mortality among children living with diabetes [ 23 , 24 ], the common evaluations include heart rate, blood pressure, sweating abnormalities, as well as the callus. However, some subjective questionnaires do not reflect any differences between the patients and healthy controls, whereas some objective function tests have not been used in children, owing to higher demand for cooperation or lack of specialized equipment [ 25 , 26 ]. Autonomic neuropathy in children with T1DM is likely underestimated due to suboptimal screening and subclinical neuropathy [ 27 ].…”
