Behçet syndrome (BS) is a multisystem vasculitis that is most active during young adulthood, causing serious disability and significant impairment in quality of life. Differences in the disease course, severity, and organ involvement between patients, depending on the age at presentation and sex, makes it impossible to determine a single management strategy. The diversity and variability in the outcome measures used in clinical trials in BS makes it difficult to compare the results or inform physicians about the best management strategy for individual patients. There is a large unmet need to determine or develop validated outcome measures for use in clinical trials in BS that are acceptable to researchers and regulatory agencies. We conducted a systematic review to describe the outcomes and outcome measures that have been used in clinical trials in BS. This review revealed the diversity and variability in the outcomes and outcome measures and the lack of standard definitions for most outcomes and rarity of validated outcome tools for disease assessment in BS. This systematic literature review will identify domains and candidate instruments for use in a Delphi exercise, the next step in the development of a core set of outcome measures that are properly validated and widely accepted by the collaboration of researchers from many different regions of the world and from different specialties, including rheumatology, ophthalmology, dermatology, gastroenterology, and neurology.

Amaç: Bu çalışmanın amacı kadınlarda Behçet hastalığının (BH) cinsel fonksiyonlar üzerine etkisi ve cinsel fonksiyonların yaşam kalitesi ve depresyon seviyeleri ile ilişkisini araştırmaktır. Yöntem: Çalışmaya Behçet hastalığı olan 33 kadın hasta ve 37 sağlıklı kadın dahil edilmiştir. Her iki gruba da Sosyodemografik Veri Formu, Kadın Cinsel Fonksiyon İndeksi (KCFİ), Beck Depresyon Ölçeği (BDÖ) ve dermatolojik hastalıklar için hazırlanmış Yaşam Kalite Ölçeği (YKÖ) uygulanmıştır. Bulgular: Hasta grubunda yaş ortalaması 37,30±8,44 (20-47) yıl ve sağlıklı kontrollerde 37,54±7,62 (22-48) yıl olup her iki grup arasında anlamlı fark yoktu (p=0,902). Cinsel disfonksiyon oranı hasta grubunda (%81,8) sağlıklı kontrol grubuna (%45,9) göre anlamlı şekilde daha yüksekti (p=0,002). Toplam KCFİ ve alt parametrelerinin (cinsel arzu, uyarılma, lubrikasyon) puan ortalaması Behçetli hasta grubunda kontrollere göre anlamlı şekilde daha düşüktü. Behçet hasta grubunda BDÖ puanı ile kadın cinsel fonksiyonları ve dermatolojik yaşam kalitesi arasında negatif yönde anlamlı şekilde ilişki mevcuttu. Sonuç: Bu çalışmada Behçet hasta grubunda cinsel disfonksiyon görülme sıklığının sağlıklı kontrollere göre anlamlı şekilde daha yüksek olduğu sonucuna varılmıştır. Kadın Behçet hastalarında depresyonun bulunmasının yaşam kalitesi ve cinsel fonksiyonlar üzerinde olumsuz etkileri olduğunu düşünmekteyiz.

Background: Approximately 40% of women and 30% of men describe sexual dysfunction, although recognition in medical settings is suboptimal, due to problems in reporting and eliciting concerns relating to sexual function and satisfaction. Screening questionnaires may help to support this aspect of clinical practice. The Arizona sexual experiences scale (ASEX) includes items that quantify sex drive, arousal, vaginal lubrication or penile erection, ability to reach orgasm, and satisfaction from orgasm. Method: We investigated the validity and other psychometric properties of the ASEX, and the findings from the populations in which it has been employed, by searching MEDLINE, EMBASE, and Google Scholar using the terms, Arizona sexual experiences scale, Arizona Sexual Experience Questionnaire, and ASEX. We eliminated duplications, letters, and papers not available in English, and grouped the remaining papers into the categories of psychometric, epidemiological, and outcomebased studies. Results: After elimination of letters and duplicates, papers not in English, and preclinical and irrelevant studies, 104 papers were analyzed. The ASEX has excellent internal consistency, scale reliability and strong test-retest reliability. Analyses of variance reveal significant differences in total ASEX scores between patients and controls and between females and males. ASEX appears to be useful in a range of clinical situations including patients with primary sexual dysfunction, specific psychiatric disorders, specific physical illnesses, and treatment emergent sexual dysfunction. Discussion: The ASEX appears to be a reliable instrument for identifying and quantifying sexual dysfunction across a range of populations in various clinical settings. Little is known about its utility in patients with anxiety disorders or relationships between ASEX scores and biological parameters.
