Este estudo transversal buscou analisar as condições bucais e o impacto na qualidade de vida de crianças e adolescentes em tratamento antineoplásico. Examinadores treinados avaliaram 51 voluntários de 2 a 18 anos selecionados por amostragem censitária, diagnosticados com neoplasia. Avaliou-se: índice de cárie (ICDAS/CPOD-ceod) fluxo salivar e capacidade tampão, presença de placa bacteriana visível (IPV), índice de sangramento gengival (ISG) e entrevista sobre hábitos de higiene oral e dieta. O questionário PedsQL® 3.0 Módulo Câncer, dimensão saúde bucal, foi aplicado para analisar impacto da saúde bucal na qualidade de vida. Os dados foram analisados no software Bioestat® 5.1 mediante aplicação do teste T student e correlação Pearson com nível de significância de 5% (p<0,05). A maioria da amostra eram do gênero masculino (76,47%), com idade média de 8,6±4,53 anos, diagnosticados com leucemia (62,74%) tratando-se com quimioterapia (92,16%). Variáveis odontológicas com CPOD médio 7,12±6,09, IPV de 49,79%, ISG de 17,91%, hipossalivação (0,56±0,34ml/min) e baixa capacidade tampão (4,02±1,15). Indivíduos com nível socioeconômico mais baixo apresentaram maior acúmulo de biofilme (p<0,05). O escore médio (83,43) do PedsQL® 3.0 mostrou baixo impacto da saúde bucal na qualidade de vida. Existiu correlação entre a prevalência de cárie e o impacto na qualidade de vida (Pearson, r= - 0,54; p= 0,0000). As crianças e adolescentes participantes da pesquisa não possuíam boa saúde bucal: hipossalivação, baixa capacidade tampão e acúmulo expressivo de placa bacteriana, fatores que aumentam o risco de desenvolvimento de doenças bucais como: cárie dentária, doença periodontal e mucosite oral, situações que podem interferir no estado de saúde geral do paciente, através do desenvolvimento de bacteremia e sepse, repercutindo negativamente no desenvolvimento e tratamento do câncer. Mesmo diante de precariedade das condições bucais, as respostas, coletadas pelo instrumento PedsQL 3.0 Módulo câncer, expressaram que a saúde bucal, com exceção da presença de cárie, não influenciou na qualidade de vida das crianças e adolescentes em tratamento antineoplásico. Torna-se necessária a implantação de estratégias educativas para melhor informar pacientes e cuidadores quanto a importância dos cuidados bucais, durante e depois do tratamento antineoplásico, para minimizar os efeitos adversos da doença e do seu tratamento.

The effect of topical chamomile in the prevention of chemotherapy‐induced oral mucositis: A randomized clinical trial

Objective To clinically assess the effectiveness of topical chamomile oral gel in the prevention of chemotherapy‐induced oral mucositis. Material and methods A parallel single‐blind randomized clinical trial conducted on 45 patients who were undergoing chemotherapy. Patients were assigned to three equal groups. Group I received conventional symptomatic treatment that included antifungal agents (Miconaz oral gel, Medical Union Pharmaceuticals), topical anesthetics, and anti‐inflammatory agent (BBC oral spray, Amoun Pharmaceutical Company) three times per day for three weeks, group II received 3% chamomile topical oral gel, whereas group III patients were given both conventional symptomatic treatment and chamomile topical oral gel. All patients were clinically assessed for pain and oral mucositis severity at three separate time intervals: 1 week, 2 weeks, and 3 weeks. Results Most patients experienced oral mucositis with more severity reported in the conventional group (grade III = 6.7%) compared to the other two groups, neither of which developed more than grade II. Mean pain scores showed no significant difference between the groups, but intragroup analysis showed that pain score increased in the conventional treatment group more than the other two groups. Conclusion Topical chamomile 3% gel has demonstrated in this study to lower the severity of the mucositis with lower pain scores compared to the other two groups.

Search strategy: Electronic searches through PubMed, MEDLINE, Cochrane Database, and Google Scholar were accomplished and a reference list of relevant articles was undertaken. Among them English-language papers with available full texts focusing on the medicinal herbs in the treatment of cancer therapy-induced oral mucositis were chosen. The keywords used were medicinal plant, chemotherapy, oral ulcer, mouth ulcer, oral mucositis, stomatitis, radiotherapy, and neoplasms.Aloe vera: The Aloe vera plant includes several active agents which contain polysaccharides, anthraquinone, lectin, superoxide dismutase (an antioxidant enzyme), glycoprotein, amino acids, vitamins C and E and minerals.This herbal agent provides antioxidant properties, COX-2 suppression and immune modulatory mechanisms. The result of a study that evaluated the oral aloe vera in the patient undergoing radiation therapy did not show any positive effects, but the quality of life scores were slightly improved [16]. Another study that was done in 2009, demonstrated that aloe vera juice is effective in reducing the incidence of mucositis in patients with radiation therapy, especially those with lower mucositis grading. Hence, aloe vera may not be able to completely prevent oral mucositis, but alleviates and reduce the progression of mucositis [17]. In a recent study by de Freitas Cuba L et al., the topical application of aloe vera has a positive effect in healing of lesions induced on the tongue of rats subjected to radiation [18]. In another study, Ahmadi A showed that aloe vera mouth wash alleviated oral mucositis due to head and neck radiation [19]. Borneol and borax powder:This agent is administered locally in oral inflammation and aphthous ulceration, which contains Borax, Borneolum, and Indigo naturalis. Borax extracts of natural borax mineral decrease genotoxic effects of heavy metal on human blood cell cultures and normalizes reduced antioxidant enzyme activities. ABSTRACTOral mucositis is an inflammatory mucosal destruction as a result of chemotherapy and/or radiation therapy, which in severe cases can impair patients' quality of life. Moreover, mucosal infection and/or systemic involvement due to compromised immunity leads to delay or discontinuation of the treatment. Many strategies and agents have been suggested for the management of this condition. Because of their lower side effects compared to chemical drugs, general interest in evaluating therapeutic effects of herbs has been increased intensively. Herbal plants apply their effect through different mechanisms of action: antioxidant, analgesic, anti-inflammatory, antifungal, antiseptic, and anticarcinogenic activity. Recently, various natural agents in plants have been noticed in mucositis, which may improve the symptoms through different interventions.
