BackgroundPharmacists in Japan currently play a key role in patient hospital care. Their responsibilities include filling prescriptions, checking a patient’s medication history, and providing appropriate information to other health care workers. More importantly, pharmacists’ interventions can also result in reductions in adverse drug reactions (ADRs) and, ultimately, in cost savings. This study aimed to determine the economic value of such interventions at a hospital in Japan.MethodsAt a single Japanese hospital, we analyzed 1452 pharmaceutical interventions by pharmacists, including recommending antibiotic dosage regimens, attending ward rounds with multidisciplinary health providers, providing drug information, and reporting ADRs. We classified the interventions into 13 categories. Using data from the PreAVOID Report by the Japanese Society of Hospital Pharmacists, along with previous studies, we estimated the cost savings of the interventions.ResultsVarious savings could be realized through appropriate interventions by hospital pharmacists. Based on the amount paid by the Pharmaceuticals and Medical Devices Agency, we calculated the cost savings associated with preventing serious ADRs as 21,400 USD ($) per case. The cost savings for recommendations related to transvenous antimicrobial therapy amounted to $1900 per patient. Pharmacists’ interventions were able to prevent 12 cases of serious ADRs.ConclusionsDetermining the economic value of pharmacists’ interventions is an important means of appraising the current role of hospital pharmacists. Our evaluation demonstrates the positive economic effects of pharmacists’ interventions in a hospital setting.

Pharmacists should play an important role in controlling the pharmaceutical management of the patients. Although the quantitative evaluations of pharmaceutical management are required systematically, no guideline is presented for patient care during lung cancer chemotherapy. We established a complete pharmaceutical management system and evaluated the performance of the system. Patients admitted to Hokkaido Keiaikai Minami-ichijo Hospital for lung cancer chemotherapy treatment from 2003.5 to 2003.11 were enrolled in this study after signing formal written consents. The pharmaceutical management system that was established involves a uniˆed system consisting of a standard care plan and worksheets by the Problem Oriented System (POS). The system can apply to inpatients for their comprehensive evaluation of pharmaceutical management. The incidence of pharmaceutical problems reported, pharmacist interventions to the physicians, and the bedside pharmaceutical management were increased signiˆcantly after applying the system. More than 98％ acceptance of interventions by pharmacists to the physicians was indicated. A high rate of intervention for adverse drug reaction (ADR) was demonstrated, due to the information about patient conditions is essential in managing ADR. The total pharmaceutical management system established is expected to give quality improvements of pharmaceutical care along with its e‹ciency.

Proposal for a New Tool to Evaluate Clinical Pharmacy Practice Based on the Percentage of Pharmaceutical Interventions that Influence Medical Treatments

Clinical pharmacy practice needs quality safeguards and proper evaluation to ensure good performance of hospital pharmacists. We propose a method for evaluating performance using the indicator of whether pharmaceutical interventions conducted in clinical practice aŠect the treatment carried out by the physician. To illustrate the usefulness of this method, we report one example of the response of a physician to our clinical practice. Weˆrst designed a worksheet on which information related to pharmaceutical interventions would be recorded. The worksheet included sections for the details of the pharmaceutical intervention, whether decisions about interventions were active (pharmacists' decisions) or passive (requests from the physician), the timing of the intervention (before or after prescription), the grounds for the intervention, and whether it had any in‰uence on the treatment, as well as a detailed record of the course of the intervention. These worksheets were used to record pharmaceutical interventions from July to December 2005. During that period, a total of 347 pharmaceutical interventions for 164 patients were recorded. Each intervention was examined from the diŠerent perspectives of type of intervention, timing (before or after prescription was issued), and reason for the intervention. Overall, it was found that 91.6％ of all interventions had some in‰uence on treatment. Because numerical results can be obtained with this method, it has the beneˆt of being an objective means of evaluating the contribution of pharmaceutical services. Furthermore, the validity of even those interventions that had no in‰uence on treatment can be examined using medical and pharmaceuticalˆndings and may improve the quality of pharmacists.
