Cel: Ocena przypadków potworniaków krzyżowo-guzicznych (sacrococcygeal teratoma, SCT) z uwzględnieniem wczesnej diagnostyki prenatalnej, wewnątrzmacicznego monitorowania płodu, występujących powikłań oraz dostępnych metod leczenia. Materiał i metody: Retrospektywny przegląd danych z piśmiennictwa dotyczący przypadków SCT diagnozowanych w latach 2009–2023 oraz opis przebiegu ciąży u 29-letniej pacjentki z rozpoznaniem SCT u płodu w I trymestrze. Badania ultrasonograficzne zostały wykonane przez jednego z autorów z wykorzystaniem aparatów Voluson E8 i E10. Wyniki: W I trymestrze ciąży trafnie rozpoznano u płodu SCT. Umożliwiło to wdrożenie prawidłowego nadzoru nad rozwojem i dobrostanem płodu oraz stanem matki, a także pomyślne ukończenie ciąży i przekazanie noworodka w stanie dobrym do dalszego leczenia chirurgicznego. Podsumowanie: Rozpoznanie SCT jest możliwe przy zastosowaniu badania ultrasonograficznego już w I trymestrze ciąży. Monitorowanie progresji guza i ocena wydolności układu krążenia to podstawowe elementy nadzoru nad płodem z SCT. Ocena czynników prognostycznych, takich jak wielkość guza, morfologia czy typ wg Altmana, umożliwia przedstawienie rodzicom rokowania dla płodu i noworodka. Podejście wielodyscyplinarne odgrywa istotną rolę w uzyskaniu jak najlepszych wyników leczenia.

Prenatal Diagnosis of Fetal Heart Failure

Fetal heart failure (FHF) is a condition of inability of the fetal heart to deliver adequate blood flow for tissue perfusion in various organs, especially the brain, heart, liver and kidneys. FHF is associated with inadequate cardiac output, which is commonly encountered as the final outcome of several disorders and may lead to intrauterine fetal death or severe morbidity. Fetal echocardiography plays an important role in diagnosis of FHF as well as of the underlying causes. The main findings supporting the diagnosis of FHF include various signs of cardiac dysfunction, such as cardiomegaly, poor contractility, low cardiac output, increased central venous pressures, hydropic signs, and the findings of specific underlying disorders. This review will present a summary of the pathophysiology of fetal cardiac failure and practical points in fetal echocardiography for diagnosis of FHF, focusing on essential diagnostic techniques used in daily practice for evaluation of fetal cardiac function, such as myocardial performance index, arterial and systemic venous Doppler waveforms, shortening fraction, and cardiovascular profile score (CVPs), a combination of five echocardiographic markers indicative of fetal cardiovascular health. The common causes of FHF are reviewed and updated in detail, including fetal dysrhythmia, fetal anemia (e.g., alpha-thalassemia, parvovirus B19 infection, and twin anemia-polycythemia sequence), non-anemic volume load (e.g., twin-to-twin transfusion, arteriovenous malformations, and sacrococcygeal teratoma, etc.), increased afterload (intrauterine growth restriction and outflow tract obstruction, such as critical aortic stenosis), intrinsic myocardial disease (cardiomyopathies), congenital heart defects (Ebstein anomaly, hypoplastic heart, pulmonary stenosis with intact interventricular septum, etc.) and external cardiac compression. Understanding the pathophysiology and clinical courses of various etiologies of FHF can help physicians make prenatal diagnoses and serve as a guide for counseling, surveillance and management.

Perinatal Outcomes of Intrauterine Interventions for Fetal Sacrococcygeal Teratoma Based on Different Surgical Techniques—A Systematic Review

Background: This study aims to evaluate the outcomes of fetal sacrococcygeal teratoma (SCT) submitted to prenatal interventions. Methods: We performed a systematic literature review of fetal SCT patients and compared the outcomes between open fetal surgery and percutaneous intervention. In addition, we also compared the results of SCT fetuses who did not undergo any surgical intervention (NI). Results: We identified 16 cases of open fetal surgery (OS), 48 cases of percutaneous fetal intervention (PI), and 93 NI patients. The survival rate was 56.2% in OS, 45.8% in PI (p = 0.568), and 71.0% in NI patients. The gestational age at delivery was earlier in cases where there was no survival compared to cases where the fetuses did survive across all evaluated cohorts (OS: p = 0.033, PI: p < 0.001, NI: p < 0.001). The gestational weeks at delivery in OS and PI fetuses were more similar; however, OS tended to be performed later on in pregnancy, and the affected fetuses had more severe presented findings. In our evaluation, we determined that the presence of fetal hydrops and cardiac failure had no significant impact on survival in SCT cases. In NI patients, polyhydramnios was much higher in fetuses who did not survive compared to their surviving cohorts (p < 0.001). Conclusions: In conclusion, gestational age at delivery can affect the short-term prognosis of fetuses affected with sacrococcygeal teratomas. Regardless of the mode of delivery or the necessity for intervention during the fetal period, monitoring for complications, including polyhydramnios, can prevent premature delivery.
