Introduction: This study aims to evaluate potentially inappropriate medication (PIM) use, the association of PIM use, and activities of daily living in 65 years and older home care patients. Method: The study included 322 patients aged 65 and over who applied to Home Care Unit. Consent was obtained from those who agreed to participate in the study. A questionnaire containing demographic data, drug use information, and Katz Daily Living Activities Index was used which was prepared by the researcher in this study. The drugs used by the patients were evaluated according to the 2015 Beers criteria and inappropriate active substances were detected. The data were defined by statistical analyzes such as mean, percentage, standard deviation, and frequency. Chi-squared test and Student's t-test were used in the analysis. The significance level was accepted as p<0.05. Results: The mean age of the patients was 80.2±8.2 years, and 63.4% (n=204) were female. 70.5% (n=227) of the elderly have three or more chronic diseases. The most common diseases were hypertension (56.2%), diabetes (30.0%), and heart failure (29.8%). 79.2% (n=255) of the group uses four or more drugs. 83.3% (n=260) of the patients get help to use the drugs. 51.6% (n=166) of the patients were fully dependent on activities of daily living and 3.4% (n=11) were fully independent, the level of dependency increases with age (p<0.05). The PIM rate was 61.5% in patients. PIM use was found to be significantly higher in those with more disease, in those who used more drugs, and in females (p<0.05). No relationship was found between inappropriate medication use and addiction level. Conclusion: In this study, it was found that more than half of home care patients aged 65 and over had inappropriate drug use. It was determined that inappropriate medication use increased with polypharmacy, and polymorbidity, and there was no correlation between addiction levels.

Bu çalışmanın amacı, 65 yaş üzeri hastaların ilaç kullanımlarını değerlendirmek ve polifarmasi hakkında bilgi sağlamaktı. Yöntem: Ocak 2013-Aralık 2013 tarihleri arasında Doğanlar Aile Sağlığı Merkezine başvuran yaşlı hastaların ((≥65 yaş) medikal kayıtları kullanılarak tek merkezli, retrospektif bir çalışma yürütüldü. Hastaların demografik özellikleri, tanıları, başvuru sıklıkları ve reçetelenmiş ilaçları değerlendirmeye alındı. Bulgular: Sağlık merkezine başvuran 218 yaşlı hasta (122 kadın, 96 erkek) çalışmaya dahil edildi. Ortalama yaş erkek hastalar için 75,52±5,4, kadın hastalar için 74,71±5,6 idi. Çoğunluğu kadın olmak üzere, hastaların yarısına yakını okur-yazar değildi. En sık görülen kronik hastalıklar sırasıyla, hipertansiyon (%48,1), koroner arter hastalığı (%27,0), diyabetes mellitus (%23,2), solunum hastalıkları (%17,2) ve hiperlipidemi (%12,8) idi. Yaşlı hastalarda çoklu ilaç kullanımı gözlendi. Hastaların %17,8'i düzenli ilaç kullanmazken, %10,5'i bir ilaç, %13,7'si iki ilaç, %11,3'ü üç ilaç, %8,7'si dört ilaç ve %38'i 5 ve daha fazla ilacı birlikte kullanıyordu. Semptomatik tedavi çok sıktı, iki hastadan birinde proton pompa inhibitörü ya da nonsteroid antiinflamatuvar ilaç kullanımı vardı (kadınlarda daha sık, p<0,05). Sonuç: Çalışmamızın sonuçları göstermektedir ki, çoklu ilaç kullanımı yaşlı hastalarda oldukça yaygındır. Yaşlılarda birlikte kullanılan ilaçların etkileri ve birbirleriyle olası etkileşimleri, tedaviye başlamadan önce dikkatli değerlendirilmelidir. İlaç kullanımı ile ilgili yapılacak daha detaylı araştırmalar, yaşlılarda ilaçla tedavide başarıyı arttırabilir.

This study sought to determine complementary and alternative medicine (CAM) methods used by older adults during the COVID-19 pandemic. The sample for this descriptive study comprised 415 adults older than 65 years who completed an online questionnaire between August and October 2020. The study used descriptive statistical methods and a χ2 test for the statistical analysis of data. In the study, 55.4% of participants had used one or more CAM methods during the pandemic. The most frequently used methods were vitamin and mineral supplements (48.5%) and herbal products (37.2%). The study determined that 43.1% of participants did not inform their health care professionals of the method they had used. It is possible that the use of CAM methods by older adults during the pandemic and the decrease in the frequency of seeking care at health care institutions could complicate the management of health problems related to the use of CAMs.
