Background and objectives: Although blood transfusion and iron chelation increase the lifespan of thalassemic patients, these patients face many complications, including osteoporosis and osteopenia. Thus, we aimed to determine prevalence rates of osteoporosis/osteopenia using Dual Energy X-Ray Absorptiometry in transfusion-dependent thalassemia patients and its relationship with chelation agents and body mass index. Methods: This retrospective study included 150 patients with transfusion-dependent thalassemia in the Thalassemia and Congenital Blood Disorders Center, Sulaimaniyah, Iraq, from December 2021 to July 2022. The participant’s information was collected from the hospital database, including sociodemographic characteristics, bone mineral density status, and the number of items used for chelation. A Dual Energy X-Ray Absorptiometry bone scan was used to check the bone mineral density. Results: The mean age of patients was 19.59±7.59, body mass index was 21.23±18.13, and most (53.3%) were females. Dual Energy X-Ray Absorptiometry results showed that 53.3% of the patients had osteopenia, 21.3% had osteoporosis, 36.7% had average bone mineral density, and 6.7% had low bone mineral density for age. In addition, there was a significant difference between the frequencies of bone density disorders in patients who used one chelating agent (46.4%) versus two chelating agents (85%) (P?0.001). Also, there was a significant difference between the incidence of bone density disorders with low and average body mass index (78%) in comparison with overweight and obese patients (8%) (P?0.001). Conclusions: There is a high prevalence of osteoporosis and osteopenia in transfusion-dependent thalassemia patients, which are more prone to trauma and bone fracture.

Metabolic syndrome is a cluster of medical conditions composed of ‎abdominal obesity, ‎hyperglycemia, hypertension, and lipid abnormalities, in which ‎each can affect bone in ‎different ways. We examine ‎the association between metabolic syndrome and metabolic bone disease ‎‎(osteoporosis) by ‎measuring adipose tissue marker (chemerin) and bone resorption ‎marker ‎‎(deoxypyridinoline) in ‎the serum of osteoporotic postmenopausal women with ‎and without ‎metabolic syndrome. ‎ A case-control study included 112 postmenopausal women from 51 to 67 years of ‎age. ‎Women were ‎selected from Osteoporosis Clinic- Al Yarmouk Teaching ‎Hospital and were ‎divided into two ‎groups: group (1)- patients group included 57 ‎postmenopausal women with ‎osteoporosis (35 ‎osteoporotic women with metabolic ‎syndrome and 22 osteoporotic women ‎without metabolic ‎syndrome) and group ‎‎(2)-control group included 55 postmenopausal women ‎without ‎osteoporosis and ‎metabolic syndrome. A serum sample was taken from each woman and ‎analysed ‎for ‎assessing chemerin, deoxypyridinoline, fasting serum glucose, and lipid profile. We found ‎a significant increase (p-value<0.001) in the mean value of serum chemerin ‎and ‎deoxypyridinoline ‎levels in patients compared to controls. ‎In ‎conclusion, chemerin is an adipose tissue marker associated with metabolic syndrome ‎and ‎may ‎have an impact on bone turn over and development of metabolic bone ‎disease ‎which ‎enhanced by the strong positive correlation between chemerin and bone ‎resorption ‎marker ‎‎(deoxypyridinoline).

Muco-adhesive gel formulations are advantageous in extending the stay at the nasal ‎absorption place, promoting drug absorption. Frovatriptan succinate (FVT) exhibits a 35% ‎oral bioavailability and undergoes hepatic metabolism, making it a viable candidate for ‎nasal delivery. This study aimed to assess novel FVT intranasal formulation for brain ‎targeting in rat animal models. A total of 78 female rats (Rattus norvegicus domestica, ‎Wister albino rats) were randomly divided into three groups: group A (considered a ‎negative control), group B (includes 36 rats given FVT IV solution), and group C (includes ‎‎36 rats given FVT binary ethosome in situ gel intranasally). Drug levels in plasma and brain ‎tissue were measured using HPLC methods. In all periods, for both brain tissue ‎concentrations of FVT and the brain-to-plasma ratio of FVT, it was significantly higher in ‎Group C compared to Group B. Nasal administration of FVT showed higher brain Tmax, ‎Cmac, and AUC compared to IV administration, with 239.83% higher accumulation of FVT ‎when nasal formulation used compared to IV administration. In conclusion, in situ gel has ‎demonstrated its efficacy in facilitating the delivery of frovatriptan succinate via the nasal ‎route. The convenience of the administration process, combined with reduced frequency of ‎administration, contributes to improved patient adherence‎‎‎.
