Evaluation of Once-Daily Vancomycin against Methicillin-Resistant Staphylococcus aureus in a Hollow-Fiber Infection Model

Abstract: For methicillin-resistant Staphylococcus aureus (MRSA) infections, data suggest that the clinical response is significantly better if the total vancomycin area under the concentration-time curve (AUC)/MIC ratio is >400. While the AUC/MIC ratio is the accepted pharmacokinetic/pharmacodynamic (PK/PD) index for vancomycin, this target has been achieved using multiple daily doses. We are unaware of a systematically designed dose fractionation study to compare the bactericidal activity of once-daily administration … Show more

“…Bacteria in all broth samples were washed twice by centrifugation and resuspension in fresh sterile saline. Viable counts were determined by manual plating of 100 l of an undiluted or appropriately diluted suspension in saline onto CAMHA plates (20,28,29) Mechanism-based modeling of antibiotic combinations. To characterize the extent, time course, and potential mechanisms of synergy for a ␤-lactam plus an aminoglycoside, mechanism-based pharmacodynamic (PD) modeling was performed.…”

“…The variances were employed in an inverseobserved-variance weighting scheme to give the best approximation to the homoscedastic assumption. As the dynamic driver for vancomycin bacterial kill for S. aureus is AUC/MIC ratio (18,19), the point estimate of clearance and its attendant 95% confidence interval were employed as the metric for comparing the six different vancomycin products.…”

“…In light of this gap in the literature, a retrospective cohort (2002 to 2013) was performed at the Detroit Medical Center to evaluate the relationship between the day 1 vancomycin exposure profile and outcomes among patients with MRSA IE. We purposefully evaluated the association between the day 1 vancomycin exposure profile and outcomes given the importance of early, appropriate ther-apy (8) and since the animal (9) and in vitro pharmacodynamics studies (10) used to define the AUC/MIC exposure target for vancomycin assessed the relationship between exposure and bacterial killing during the first 24 h of therapy. To accomplish the study objectives, a validated Bayesian approach was used to estimate the vancomycin exposure profile with limited PK samples (11).…”
