Evaluation of lung function and deposition of aerosolized bronchodilators carried by heliox associated with positive expiratory pressure in stable asthmatics: A randomized clinical trial

Abstract: While administration of medical aerosols with heliox and positive airway pressure are both used clinically to improve aerosol delivery, few studies have differentiated their separate roles in treatment of asthmatics. The aim of this randomized, double blinded study is to differentiate the effect of heliox and oxygen with and without positive expiratory pressure (PEP), on delivery of radiotagged inhaled bronchodilators on pulmonary function and deposition in asthmatics. 32 patients between 18 and 65 years of ag… Show more

“…39,40 Methods to reduce airway closure, such as application of positive expiratory pressure, may thus have benefit for improving inhaled drug deposition. 41 Our data reveal a median closing index of 0.67 among a large, diverse group of patients with asthma. This value is very similar to the mean value (0.60) found by Chapman et al in a much smaller (n = 62) groups of patients with asthma.…”

“…In the multivariable linear regression model, a higher closing index was associated with higher age (10 year increments) (0.04, 95% CI = 0.02, 0.05, P < 0.005) and obesity (0.07, 95% CI = 0.03, 0.10, P < 0.001) after controlling for race, sex and study ( Table 3). Age of asthma onset and baseline use of 31 (20,41) 41 (26,50) 37 (26,47) 11 (9,14) 22 (12,38) <0.001…”

“…When Heliox is used to drive bronchodilator nebulization, deeper lung delivery of the agent is seen [98]. Improved FEV 1 is seen using Heliox-driven bronchodilator nebulization compared to using pure oxygendriven systems [99,100]. In children (ages 2-21 years) using Heliox-powered albuterol, nebulization failed to demonstrate shorter hospitalizations in moderate-to-severe AEA compared to oxygen-driven albuterol nebulization [101].…”
