Evaluation of l -3-[ 123 I]iodo-α-methyltyrosine SPET and [ 18 F]fluorodeoxyglucose PET in the detection and grading of recurrences in patients pretreated for gliomas at follow-up: a comparative study with stereotactic biopsy

Abstract: Based on the results of stereotactic biopsy, we evaluated in a prospective fashion the efficiency of l-3-[123I]iodo-alpha-methyltyrosine-single-photon emission tomography (SPET) and [18F]fluorodeoxyglucose (FDG) positron emission tomography (PET) in the detection and grading of recurrences in patients previously treated for gliomas. The patient population comprised 30 individuals, nine with astrocytomas of grade II, ten with astrocytomas of grade IV, three with oligoastrocytomas of grade II, six with oligodend… Show more

“…In a non-randomized study evaluating outcome after re-irradiation with or without temozolomide chemotherapy in 44 post-surgical patients with recurrent high grade glioma, patients who underwent SPECT with [ 123 I]IMT or PET with [ 11 C]MET in addition to MR and CT as part of their pretreatment planning had longer median survival times (9 vs 5 months) than those who were imaged with MR and CT alone [107]. While encouraging, valida- [94,[107][108][109][110][111]. Although [ 123 I]IMT is an effective brain tumor imaging agent, the specificity of [ 123 I] IMT for brain tumors is not perfect.…”

“…[ 123 I]-IMT SPECT allows a particularly good distinction between residual/recurrent gliomas and radionecrosis several weeks after radiotherapy [3,33]. To date, the early changes of [ 123 I]-IMT uptake following irradiation have not yet been evaluated either in vitro or in vivo.…”

“…When considering both low-and high-grade gliomas, [16][17][18][19][21][22][23]27,32,33,40,41 sensitivity ranged between 0.23 and 0.95 and specificity ranged between 0.17 and 1.0. For high-grade gliomas, [16][17][18][19]27,34,35,39,41 sensitivity ranged between 0.18 and 1.00 and specificity ranged 0.25 and 1.0.…”
