Reproductive efficiency is a critical component of a successful dairy operation and acts as an important component of a profitable dairy farm, whereas reproductive inefficiency is one of the most costly problems facing the dairy industry today. Reproductive problems occur frequently in lactating dairy cows and can dramatically affect reproductive efficiency in a dairy herd. Some of the most common problems include twinning, dystocia, abortion, stillbirth, retained placenta and metritis. These are diverse disorders that are similar in that they all can result in impaired reproductive function. Deciding whether to breed, treat, or cull dairy cows exhibiting one or more of these reproductive problems is a challenge for both veterinarians and dairy producers. In addition, there is considerable controversy among dairy scientists and bovine practitioners regarding the economic impact of these problems in a dairy operation and the most effective management or therapeutic intervention for treating them. Because of this controversy, dairy managers should focus on prevention and control of risk factors associated with each problem rather than on prescriptive therapeutic interventions. Dairy producers should work closely with their herd veterinarian to develop such management strategies and discuss appropriate interventions when necessary (Fricke, 2001).Low fertility reduces the profit by decreasing the average milk production and the number of calves per cow per year. ABSTRACT. In this review, updated and precise definitions of the most common postpartum uterine diseases in dairy cows are provided. An aberrant uterine environment at inappropriate stages of the reproductive cycle inflicts damage to gametes and zygotes, impairing the reproductive performance of dairy cows. This involves major economic losses for the milk production unit. Consequently, an accurate diagnosis of postpartum uterine diseases is indispensable for practitioners to set up a prompt and efficient treatment. This review furthermore emphasizes on the new perspectives regarding diagnosis and treatment of subclinical endometritis, a highly prevalent uterine disease that is often overlooked by practitioners while causing major reproductive problems. Based on a more profound clinical understanding of the postpartum uterine disease complex, practitioners will be able to better use the available diagnostic tools and therefore apply a more efficient therapeutic approach. RESUMEN. El objetivo de este artículo de revisión es proporcionar definiciones actualizadas y precisas de las enfermedades uterinas posparto más comunes en vacas lecheras. Un ambiente uterino hostil en etapas inapropiadas del ciclo reproductivo inflige daño a los gametos y cigotos, lo que perjudica el rendimiento reproductivo de las vacas lecheras. En este contexto, una menor fertilidad contribuye negativamente al éxito económico de la unidad de producción lechera. En consecuencia, un diagnóstico adecuado de la enfermedad del útero en el posparto es indispensable para que los profesionales veterinarios puedan establecer un tratamiento con prontitud y eficacia. Esta revisión hace hincapié en las nuevas perspectivas de diagnóstico y tratamiento de la endometritis subclínica, una patología uterina de alta prevalencia que a menudo los profesionales pasan por alto. The objective of the study was to determine the effect of puerperal metritis prevalence in high yielding Holstein cows on productive (305 days, actual milk yields), reproductive (days to first estrus, days open and services per conception) variables and culling rates. Throughout the study, 2885 dairy records were collected from high yielding private Holstein dairy farm (average milk yield was up to 9000 kg) for 3 successive lactation seasons. Dairy cows were classified according to their uterine affection into two groups, healthy and those with puerperal metritis. Dairy cows which affected with puerperal metritis were diagnosed during the first 3 weeks of lactation. Metritic cows were diagnosed and treated at the discretion of the herd veterinarian. Affected dairy cows were grouped into four groups according to time of the puerperal metritis attack during the first 21 Days In Milk (DIM) as (<5; 5-10; 10-15 and >15 DIM, respectively). Moreover, the dairy females were grouped into 3 groups according to frequency of puerperal metritis of once, twice and triple, respectively. Dairy cows were also grouped according to their calving season (winter, spring, summer and fall calving seasons). Productive, reproductive variables and culling due to puerperal metritis were detected from the on-farm record system (DairyComp305). Results of this study revealed that, puerperal metritis had bad effects on high yielding cows productive and reproductive variables, that decreased the 305 milk yield, increased days to first estrus, days open and number of services per conception. Moreover, the occurrence of puerperal metritis increased during the warm months of spring and summer seasons. Also, the occurrence of such disease was increased during the first 10 days of postpartum as (89.98%) of the affected cases occurred within this period. Care must be directed to dairy cows which had a history of puerperal metritis as these cases had a greater tendency for recurrence. Also, the re-attacked cows with puerperal metritis had a great chance to be culled from the dairy herds, as they reached to (53.33% and 100%) for the second and third attack, respectively.
