Purpose Magnesium supplementation is an effective protective method against cisplatin-induced nephrotoxicity (CIN); however, there are few reports regarding the mechanism of its nephroprotective effect. The aim of this study was to determine whether premedication with intravenous magnesium prevents CIN and to determine the relationship between its nephroprotective effect and serum magnesium level. Methods Fifty-eight patients with head and neck cancer who received cisplatin, docetaxel, and 5-fluorouracil (DCF) were retrospectively investigated. Grade 2 or more serum creatinine elevation was defined as CIN. The incidence of CIN was compared between a magnesium sulfate (20 mEq, 2.46 g) premedication group and a non-magnesium group during the first cycle and in all cycles. Results CIN did not occur in any patients receiving magnesium premedication but did occur in 5 of 29 patients during the first cycle and in 6 patients during all subsequent cycles in patients who did not receive magnesium premedication. Furthermore, the variation of creatinine clearance was significantly worse in the non-magnesium group than in the magnesium premedication group from baseline. There was no difference in adverse effects or response rate between the two groups. Univariate analysis suggested that magnesium premedication significantly reduced the risk of CIN. On the other hand, serum magnesium depletion was seen in both groups to equal degrees despite supplementation. Conclusion Intravenous magnesium premedication has a protective effect on cisplatin-induced nephrotoxicity without the influence on the serum magnesium level. Magnesium premedication is a simple nephroprotective method that does not influence other adverse effects or rate of response to chemotherapy.

La combinación con radioterapia en ciertos tumores en que el tracto gastrointestinal forma parte del campo de tratamiento puede producirlo. Caso clínico: Se presenta el caso de una mujer de 35 años de edad diagnosticada de cáncer de cérvix en tratamiento con quimioterapia y radioterapia concomitante. Debido a la extensión del tumor y a la afectación ganglionar, el campo de tratamiento radioterápico era amplio, con dosis grandes sobre el intestino delgado. Durante el tratamiento, la paciente presentó una diarrea importante debido al déficit de magnesio. Resultados: El estudio de la hipomagnesemia no es habitual en la práctica clínica, pero debe contemplarse en aquellos pacientes que estén en tratamiento con esquemas quimioterápicos que contenga cisplatino y radioterapia pélvica ampliada. Discusión: Conocer esta afección y proporcionar un tratamiento adecuado puede evitar empeoramientos en la calidad de vida de los sujetos afectos, y la posible finalización precoz del tratamiento por mala tolerancia. Introduction: Magnesium deficiency is a common entity in cancer patients owing to cisplatin-containing chemotherapeutic schemes. Combination with radiotherapy in certain tumors where the gastrointestinal tract is part of the treatment field can elicit this entity. Clinical case: The case is presented of a 35-year-old woman diagnosed with uterine cervix cancer on treatment with concomitant chemotherapy plus radiotherapy. Owing to the tumor extension and lymph node involvement, the radiation treatment field was broad, with important doses at the level of the small intestine. During treatment, the patient had important diarrhea due to magnesium deficit. Results: Testing for hypomagnesemia is not common in clinical practice, but it should be contemplated in patients on treatment with cisplatin-containing chemotherapeutic schemes and extended-field pelvic radiotherapy. Discussion: Knowing this entity and providing adequate treatment can prevent quality of life deterioration in affected subjects and possible early treatment discontinuation due to poor tolerance.
