Evaluation of Insulin Sensitivity and β-Cell Function Indexes Obtained From Minimal Model Analysis of a Meal Tolerance Test

Abstract: Modeling analysis of glucose, insulin, and C-peptide following a meal has been proposed as a means to estimate insulin sensitivity (S i ) and ␤-cell function from a single test. We compared the model-derived meal indexes with analogous indexes obtained from an intravenous glucose tolerance test (IVGTT) and hyperglycemic clamp (HGC) in 17 nondiabetic subjects (14 men, 3 women, aged 50 ؎ 2 years [mean ؎ SE], BMI 25.0 ؎ 0.7 kg/m 2 ). S i estimated from the meal was correlated with S i estimated from the IVGTT and… Show more

“…In our beta cell model, the parameter that is related to acute insulin response, namely, rate sensitivity, was not significantly affected by treatment (Table 2). This could be due to the insufficient power of the OGTT to detect changes in acute insulin secretion [21], as also indicated by the large variability of the rate sensitivity parameter (Table 2) and its modest reproducibility [16], or to the intrinsic differences between the i.v. glucose test and the OGTT [19].…”

“…This component (Fig. 1) creates a response analogous to the ␤-cells firstphase insulin secretion (7,(12)(13)(14) and has been used to aid many closed-loop intravenous insulin delivery systems (15)(16)(17)(18)(19), including those of the Biostator (20 -22). However, the system being evaluated here relies on subcutaneous insulin delivery, which adds a substantial delay to the appearance of insulin in plasma.…”

“…Subjects had been treated with continuous subcutaneous insulin infusion (CSII) using Lispro insulin (Lilly, Indianapolis, IN) for at least 6 months before study enrollment and were required to have an HbA 1c Ͻ9%. Data from a previously published study (7) characterizing insulin secretion over a 24-h period in nondiabetic subjects are included for comparison of the glucose profiles (n ϭ 17) obtained with a similar diet. The study was approved by the University of California, Los Angeles Institutional Review Board, and all patients gave written informed consent.…”

“…In particular, the model proposed in ref. 9, validated with the hyperglycaemic clamp (13), provides indices of the control exerted on insulin secretion by glucose and glucose rate of change. Estimation of insulin sensitivity is more complicated from an oral than an intravenous glucose tolerance test because the rate of glucose absorption is an additional variable to be modeled.…”
