Background. Liraglutide (a glucagon-like peptide 1 analog) was used for the treatment of type 2 diabetes (T2DM) which could produce glucose-dependent insulin secretion. Aim. The aim was to investigate whether liraglutide could improve myofibril and mitochondria injury in skeletal muscle and the mechanisms in diabetic KKAy mice. Method. We divided the male KKAy mice into 2 groups: liraglutide group (250 μg/kg/day liraglutide subcutaneous injection) and model group; meanwhile, the male C57BL/6J mice were considered as the control. After 6 weeks, the ultrastructure of skeletal muscle was observed by electron microscope. The gene expressions of protein tyrosine phosphatase 1B (PTP1B), phosphatidylinositol 3-kinase (PI3K), and glucose transporter type 4 (GLUT4) were determined by real-time PCR. The protein levels of the above molecules and phospho-Akt2 (p-Akt2) were measured by Western blot. Results. Liraglutide significantly ameliorated the injury of mitochondria by increasing the number (+441%) and the area (+113%) of mitochondria and mitochondrial area/100 µm2 (+396%) in skeletal muscle of KKAy mice. The results of real-time PCR and Western blot showed that liraglutide downregulated PTP1B while it upregulated PI3K and GLUT4 (P < 0.01). The protein level of p-Akt2/Akt2 was also increased (P < 0.01). Conclusion. These results revealed that liraglutide could improve myofibril and mitochondria injury in skeletal muscle against T2DM via PTP1B and PI3K/Akt2 signaling pathway.

Background: Diabetic foot is the most serious and disabling long term complication of diabetes, adversely affecting the health related quality of life (HRQoL). Objectives: To evaluates the impact of patient-education (PE) on HRQoL of diabetic foot ulcer (DFU) patients. Methods: This randomized controlled study on the effect of PE on improvement of HRQoL, conducted in Kasturba Hospital, Manipal, India consisted of 135 DFU patients with 65 subjects in the control group (CG) and 70 subjects in the intervention group (IG). RAND-36 questionnaire was employed for evaluating HRQoL scores of DFU patients in both groups at baseline and after six months of PE. Results: Subjects in the both IG and CG reported poor HRQoL scores, on all the eight subscales at baseline. After six months of PE, HRQoL improved substantially in IG with respect to CG and IG at baseline (p < 0.05). Likewise, in both CG and IG, the physical component summary scale (PCS) scores (24.1 ± 6.6 vs 25.6 ± 7.3) and the mental component summary scale (MCS) scores (27.3 ± 5.4 vs 28.8 ± 7.1) were similar and poor before PE. However, six months post PE, in IG, the Both PCS and MCS scores increased dramatically from 25.6 ± 7.3 to 42.9 ± 9.7 and 28.8 ± 7.1 to 48.8 ± 8.4 (p < 0.05) respectively. Conclusion: The remarkable improvement in the HRQoL in IG at six months follow-up suggests that PE significantly improves HRQoL in DFU patients. Overworked physicians and paucity of trained doctors in resourcepoor settings obligate task-sharing with non-physician healthcare providers.

Diabetes mellitus is a major public health problem that can cause a number of serious complications. Foot ulceration is one of its most common complications. Poor foot care knowledge and practices are important risk factors for foot problems among diabetics. The present study was undertaken in the diabetes outpatient department of a tertiary care hospital to assess the practices regarding foot care in diabetes, find out the determinants of foot ulcer in diabetics, and offer suggestions to improve care. After informed consent, a total of 124 diabetics were interviewed to collect all relevant information. The diabetic foot care practice responses were converted into scores and for the sake of analysis were inferred as poor (0–5), fair (6-7), and good (>7) practices. Of the study population, 68.5% (85/124) consisted of men. The disease was diagnosed within the last 5 years for 66% (81/124) of the study participants. Of the study subjects, 83% (103/124) were on oral hypoglycemic agents (OHAs), 15.3% (19) on insulin, and 2 on diet control only. Among them about 18.5% had a history of foot ulcer. 37.9% reported using special slippers, 12% diabetics used slippers indoors, and 66.9% used slippers while using toilet. Of the study subjects, 67.8% said that feet should be inspected daily. 27.4% said they regularly applied oil/moisturizer on their feet. There is a need on part of the primary or secondary physician and an active participation of the patient to receive education about foot care as well as awareness regarding risk factors, recognition, clinical evaluation, and thus prevention of the complications of diabetes.
