Evaluation of evidence-based practice of catheter associated urinary tract infections prevention in a critical care setting: An integrative review

Abstract: Background and objective: An estimated 449,300 catheter-associated tract urinary infection (CAUTI) incidents affecting Americans and 13,000 CAUTI-related deaths in the United States every year. The purpose of the review was the appraisal and integration of the best evidence practice for preventing CAUTI interventions and strategies to guide safety and quality initiatives in order to improve patient care. “…As a result of the increased risk of post-stroke infections caused by strokeinduced immunosuppression, stroke patients with indwelling urinary catheter are more susceptible to CAUTI. Furthermore, both urinary tract infection and indwelling urinary catheter use are linked to poorer outcomes in stroke patients (Henry, 2018;John et al, 2018).…”

“…6,18,39 Amerika Birleşik Devletleri'nde mesane paketi uygulaması ile kateter ilişkili idrar yolu enfeksiyonu insidansının %30 azaldığını gösteren birçok çalışma olduğu, hatta bir çalışmada mesane paketi uygulamasının kateter ilişkili idrar yolu enfeksiyon gelişimini %50 oranında önlediği ifade edilmektedir. 39 Nörolojik yoğun bakım ünitesinde yapılan tek merkezli bir çalışmada; mesane paketi (kateter uygulamasından kaçınma, sterilitenin sürdürülmesi, ürün standardizasyonu, kateterin erken çıkarılması) uygulamasıyla, kateter kullanımının %100'den %73'e düştüğü belirtilmektedir. Aynı çalışmada, bir günde 1,000 kateterde, kateter ilişkili idrar yolu enfeksiyonu gelişiminin 13,3'ten 4,0'a düştüğü gösterilmiştir.…”

“…CAUTI preventive strategies are known and recommended in several evidence-based clinical guidelines [ 3 , 6 ]. The strategies include inserting the UC only when appropriate, aseptic insertion technique, proper maintenance, and timely removal of the IUC [ 17 , 20 , 21 ]. Implementation of the recommendations results in a significant reduction in CAUTI rates [ 4 , 16 ].…”
