Introduction: Pelvic congestion syndrome is defined as the presence of varicose changes in the ovarian veins and venous plexuses associated with chronic pelvic pain syndrome. The main problem with pelvic congestion syndrome is the low awareness of medical staff about this syndrome and the resulting very low recognition. State of knowledge: Pelvic congestion syndrome is an important cause of chronic pelvic pain. The diagnosis is based on imaging examination with ultrasound as a first-line choice and correlation with clinical presentation. The aetiology is multifactorial and may result from a combination of genetic predisposition, anatomical abnormalities, hormonal, mechanical factors with venous wall weakness, reverse blood flow and hypertension. The incidence is strongly related to the number of pregnancies. Conclusions: Pelvic congestion syndrome is one of the possible causes of chronic pelvic pain, thus differential diagnosis should necessarily include venous insufficiency during interviewing, clinical and ultrasound examination of patients. Among several treatment methods, intravascular embolization is a minimally invasive, effective and safe, currently recognised as a method of choice in treatment.

This paper discusses an essential interdisciplinary issue, pelvic congestion syndrome (PGS). PGS is a venous disorder with gonadal vein and/or pelvic venous plexus dilation that results in pelvic venous plethora. The authors address PGS prevalence, causes, underlying pathophysiological mechanisms, major clinical manifestations, diagnostic tools, and treatment approaches. Pharmacotherapy (gonadotropin-releasing hormone agonists, danazol, combined oral contraceptives, progestins, phlebotonics, etc.) is the first-line treatment that provides only a temporary effect. Image-guided vascular surgery (embolization, stenting) is of particular importance among surgical interventions. Surgical technique and critical results of the studies on the efficacy of gonadal vein embolization (from 47% to 100%) are described. The paper details major complications of surgical interventions and highlights issues requiring further researches, i.e., the effects on female fertility, predicted full recovery considering compromised somatic and/or gynecological anamnesis. Finally, the importance of timely diagnosis, greater awareness of primary care physicians on the signs and symptoms of this disorder, and a need to refer these women to vascular surgeons are addressed. Lower extremity venous diseases or insufficiency include clinically deteriorating conditions with morphological and functional alterations of the venous system including venous hypertension, vascular wall structural abnormality, and venous valvar incompetency in association with a inflammatory process . In fact the same pathophysiological processes are the main underlying mechanisms of other venous insufficiencies in different vascular territories such as varicocele, pelvic varicosities or congestion syndrome and hemorrhoids.Regarding the anatomical continuity of lower extremity venous system, urogenital system (pampiniform plexus in male and broad ligament and ovarian veins in female) and anorectal venous system, it is reasonable to expect common symptoms such as pain, burning sensation, pruritis, swelling, which arises direclty from the involved tissue itself. High coexistence rate of peripheral varicose vein, varicocele/pelvic congestion syndrome and hemorrhoid between eachother undelines not only the same vascular wall abnormaility as an underlying etiology but also existence of common symptoms originating from the involved tissue in dilating venous disease. Accordingly, it might be reasonable to query the common symptoms of venous dilating disease in other venous vascular region in patients with complaints of any particular venous territory.
