IntroductionMany studies have investigated the usefulness of cytokeratin 19 fragments (CYFRA 21-1) in pleural fluid for the differential diagnosis of benign (BPE) and malignant pleural (MPE) effusions. In the present meta-analysis, the reported studies on the diagnosis between CYFRA 21-1 and pleural effusion were assessed to summarize the diagnostic characteristics of CYFRA 21-1 in Chinese patients.Material and methodsThe data sources from the creation of each database up to January 2011 included Medline, Chinese National Knowledge Infrastructure, EMBASE, Cochrane Library, and bibliographies of review and original articles. Through a systematic literature search for publications, the data from 22 studies were summarized based on their discussions on the result of the CYFRA 21-1 assay in pleural effusion and differential diagnosis evaluation in the Chinese population.ResultsA total of 22 studies were available for analysis, and the high CYFRA 21-1 level in MPE was significantly associated with risk for lung cancer (standardized mean difference [SMD] = 1.65, 95% confidence interval [CI] = 1.48–1.82, Z = 18.97, p < 0.00001) compared with BPE. The CYFRA 21-1 level in pleural effusion (13 studies) was significantly higher than that in serum (SMD = 1.10, 95% CI = 0.71–1.48, Z = 5.59, p < 0.00001). The risk for squamous cell carcinoma (SCC) for CYFRA 21-1 was 1.03 (95% CI = 0.64–1.42, Z = 5.15, p < 0.00001) compared with that of adenocarcinoma (8 studies). The sensitivity of CYFRA 21-1 reported in the articles ranged from 46% to 94%, and the specificity ranged from 57% to 100%. The summary measure of the test characteristics derived from the summary receiver operating characteristic curve was 81% for both sensitivity and specificity (17 studies).ConclusionsThe measurement of pleural CYFRA 21-1 is likely to be a useful diagnostic tool for the confirmation of MPE.

Comparative proteomic analysis of exhaled breath condensate in patients with lung carcinoma using high resolution mass-spectrometry SummaryAnalysis of exhaled breath condensate (EBC) is a promising non-invasive method to diagnose respiratory diseases. Most researchers emphasize the importance of proteomic analysis of EBC for early diagnosis of certain respiratory diseases including lung cancer. This study was aimed at identification of potential biomarkers of neoplastic disorders in EBC of patients with lung cancer using high-performance liquid chromatography and high resolution mass-spectrometry. The study involved 26 patients with lung carcinoma (21 males, 5 females, mean age 57 ± 12 years) and 23 healthy non-smokers (19 males, 4 females, mean age 30 ± 7 years). EBC samples were collected using a disposable portable condenser R-Tube. The most of proteins identified (65 %) belonged to keratin family including type 1 (1; 2; 5 and 6А) and type 2 (9; 10; 14; 16 and 17) cytoskeletal keratins and transport proteins (albumin, lipocalin-1). Keratin family proteins (5, 6 and 14) prevailed in lung cancer patients compared to controls (p < 0.05). Other 6 proteins were also detected predominantly in lung cancer patients including β-subunit and α-subunit of haemoglobin, nuclear ubiquitous casein (NUCKS), high-mobility group proteins (HMG-I/HMG-Y), and lactoferrin. Most of these proteins could be used as a diagnostic panel to detect lung cancer. Further investigations are needed to estimate diagnostic values of these biomarkers and their role in pathogenesis of lung cancer.

Tumor markers are substances that are found in blood, urine, or body tissues and that are used as indicators for tumors; elevated tumor marker values can indicate the presence of cancer, but there can also be other causes. We have used a medical database compiled at the blood laboratory of the General Hospital Linz, Austria: Several blood values of thousands of patients are available as well as several tumor markers. We have used several data based modeling approaches for identifying mathematical models for estimating selected tumor marker values on the basis of routinely available blood values; in detail, estimators for the tumor markers AFP, CA-125, CA15-3, CEA, CYFRA, and PSA have been identified and are analyzed in this paper. The documented tumor marker values are classified as "normal" or "elevated"; our goal is to design classifiers for the respective binary classification problems. As we show in the results section, for those medical modeling tasks described here, genetic programming performs best among those techniques that are able to identify nonlinearities; we also see that GP results show less overfitting than those produced using other methods.
