Evaluation of corneal topographic changes and surgically induced astigmatism after transconjunctival 27-gauge microincision vitrectomy surgery

Abstract: Minor corneal surface and induced astigmatic changes are expected to result in rapid visual rehabilitation after pars plana vitrectomy with the 27-g MIVS system.

“…[16,17] In contrast, many other reports have stated that there were no changes in the postoperative corneal shape that occurred when using 23-, 25-, and 27-gauge small incision vitrectomy. [1][2][3][4][5] In our current study, changes in the higher-order irregular astigmatism component of the corneal shape were even transiently observed in the nongas group on the day after the operation. However, it should be noted that differences associated with the corneal shape measuring devices used may have contributed to this result.…”

“…In recent years, numerous corneal shape measuring apparatuses have been developed, including the Placido disk type corneal shape analysis, Scheimpflug-type corneal shape analysis, and anterior ocular optical coherence tomography (OCT). [ 3 ] Each corneal shape measuring apparatus has its own unique characteristics. As corneal shape measurements using anterior segment OCT are less affected by the condition of the tear film, this method is suitable for use in accurately measuring the postoperative corneal shape, since the postoperative tear film condition has been shown not to be stable.…”

“…It has been well documented in the literature that utilization of smaller-diameter instruments with self-sealing, transconjunctival scleral wounds provide some advantages such as decreased postoperative pain and inflammation, decreased induced astigmatism, and improved patient comfort. [2][3][4][5] On the other hand, despite these improvements, some complications are still available following PPV in which cataract development or acceleration is one of the most common.…”
