Recent advances in technology have allowed the production and the coherent detection of sub-ps pulses of terahertz (THz) radiation. Therefore, the potentialities of this technique have been readily recognized for THz spectroscopy and imaging in biomedicine. In particular, THz pulsed imaging (TPI) has rapidly increased its applications in the last decade. In this paper, we present a short review of TPI, discussing its basic principles and performances, and its state-of-the-art applications on biomedical systems.

Introduction: Terahertz (THz) waves with frequencies in the range of 0.1 to 10 THz are electromagnetic radiation with growing applications in various fields of science and technology. Attractive features of this radiation have brought out many novel possibilities for medical diagnostics and treatments with considerable advantages compared to other conventional methods. Methods: In this paper, we present a review of more recent reports on practical applications of THz radiation for diagnostic, biosensing and clinical treatments. The review includes the diagnosis of breast, skin, mouth, cervical, lungs, small intestine, prostate, colon, and stomach cancers, the evaluation of biomolecules, the detection of genetic mutations, the determination of burn depth, the diagnosis of tooth decay, diabetes, and emotional-psychological states, the evaluation of corneal water to diagnose visual diseases, and wound healing monitoring. Further, it embraces the use of THz therapy in reducing the size of the tumor, treating skin cancer, and healing burn wounds, cardiovascular disease, corneal epithelium, angina, and THz heating. Results: This review has emphasized the capabilities of THz waves as a novel tool for future clinical diagnostics and treatments. Conclusion: The paper provides a comprehensive understanding of the feasible potential application of THz waves for clinical purposes and its advantages in comparison with other conventional tools.

Currently, cancer is a large contributing factor in the increased mortality rates and at present the predictions are estimating an increased trend. The conventional medical cancer imaging modalities, for example X-Ray and Computed Tomography use ionizing radiation which is not tissue friendly for repeated assessments. The Terahertz (THz) cancer imaging offers novel opportunities for non-ionizing, non-invasive and early cancer detection, or diagnosis as well as improved cancer patient treatment follow-ups. In this review, a broad overview is given on the potential of THz radiation-based imaging and sensing as a technique for detection of various cancers cells. The THz radiation dynamics and interaction mechanisms with biological systems as well as parameter extraction and modelling for the observed THz image contrast are studied. The experimental studies on THz imaging and sensing are investigated with the goal approach to investigate the ex vivo, in vitro, and in vivo observations. The use of advanced analytic algorithms, specifically deep learning, is proposed for improved detection, discrimination of complex tissue with overlapping dielectric properties and development of clinical decision support systems. Research gaps in the THz imaging studies are identified based on recent trends, latest strategies suggested and the roadmap for future research direction provided.
