Amaç: Çalışmamızda önce izleyip sonra kuru kutu ve deney hayvanları üzerinde çalıştıktan sonra laparoskopi deneyimi olan cerrahlar gözetiminde yapılan ilk laparoskopik cerrahi prosedürlerin etkinlik ve güvenliğini değerlendirmeyi amaçladık. Gereç ve Yöntem: Tek cerrahın (A.A) laparoskopik eğitimi sonrası Mayıs 2015 ile Eylül 2016 tarihleri arasında yapmış olduğu 21 laparoskopik cerrahi prosedürün verileri, dosya ve bilgisayar kayıtlarından retrospektif olarak elde edilip değerlendirilmiştir. Bulgular: Çalışmamıza dahil edilen hastaların 13'ü kadın 8'i erkek olup yaş ortalaması 53±18 yıl ve vücut kitle indeksleri 27,9±3,67 kg/m2 idi. Ortalama operasyon süresi 157,76±51,14 dakika olup ortalama sonda ve dren çekilme süreleri sırasıyla 18,1±7,2 saat, 42,1±14,7 saat olarak tespit edildi. Total komplikasyon oranı %4,76 idi. Vakaların biri hariç hiçbirinde açık cerrahiye geçilmedi ve kan transfüzyon ihtiyacı duyulmadı. Tartışma: Temel teorik eğitim, kuru laboratuvar ve hayvan üzerindeki çalışma sonrası uygun hasta seçimi ve laparoskopi tecrübesi olan cerrah gözetiminde yapılan cerrahilerle laparoskopiye başlanılması eğitim sürecini hızlandıracağı, komplikasyonları önemli ölçüde azaltacağı ve cerrahin kendine olan güvenini arttıracağı kanaatindeyiz.

Objective: Safety checklists have become an integral part of surgical practice. The aim of this quality improvement study was to identify and eliminate weaknesses within safety checklists for shockwave lithotripsy (SWL), flexible cystoscopy and transrectal ultrasound-guided (TRUS) biopsy of prostate. Subjects and methods: A root cause analysis was carried out into a case of incorrect side SWL treatment. Furthermore, we carried out a survey amongst staff regarding the suitability of our flexible cystoscopy and TRUS biopsy checklists. Following the introduction of new checklists, a satisfaction survey was sent to staff. Results: The initial staff survey regarding flexible cystoscopy and TRUS biopsy identified at least three irrelevant questions on the checklist, with a further six with low-moderate importance. A single checklist was created for flexible cystoscopy and TRUS biopsy, whilst a separate checklist was developed for SWL. The follow-up satisfaction survey rated the new checklists as 'satisfactory' (82%) or 'very satisfactory' (18%). Conclusion: Appropriate checklists, with questions relevant to the procedure being undertaken will lead to greater staff satisfaction and engagement. This may lead to better staff participation, which is likely to reduce errors. Level of evidence: 3.

ObjectiveUrology practice has undergone several changes in recent years mainly related to novel technologies introduced. We aimed to get the residents’ perspective on the current residency program in Israel and propose changes in it.MethodsA web-based survey was distributed among urology residents.Results61 residents completed the survey out of 95 to whom it was sent (64% compliance). A total of 30% replied that the 9 months of mandatory general surgery rotation contributed to their training, 48% replied it should be shortened/canceled, and 43% replied that the Step A exam (a mandatory written certifying exam) in general surgery was relevant to their training. A total of 37% thought that surgical exposure during the residency was adequate, and 28% considered their training “hands-on.” Most non-junior residents (post-graduate year 3 and beyond) reported being able to perform simple procedures such as circumcision and transurethral resections but not complex procedures such as radical and laparoscopic procedures. A total of 41% of non-junior residents practice at a urology clinic. A total of 62% of residents from centers with no robotics replied its absence harmed their training, and 85% replied they would benefit from a robotics rotation. A total of 61% of residents from centers with robotics replied its presence harmed their training, and 72% replied they would benefit from an open surgery rotation. A total of 82% of the residents participated in post-graduate courses, and 81% replied they would engage in a clinical fellowship.ConclusionGiven the survey results we propose some changes to be considered in the residency program. These include changes in the general surgery rotation and exam, better surgical training, possible exchange rotations to expose residents to robotic and open surgery (depending on the availability of robotics in their center), greater out-patient urology clinic exposure, and possible changes in the basic science period.
