Aim: Blood glucose levels, cardiac troponin (cTn) values within the first 12 hours, ejection fraction (EF), bundle branch block and Gensini scores of patients who were admitted to the emergency department for acute inferior myocardial infarction (AIMI) were compared based on the bundle branch of the occluded right coronary artery (RCA) with regard to prognosis. Materials and Methods: The data of the study were acquired through retrospective review of 212 patients with AIMI (48 women, 164 men; median age=64 years) who applied to the emergency department due to chest pain and who were hospitalized in cardiology clinic between January 2012 and December 2015. The patients were divided into five RCA groups based on the angiography results. The level of mortality and three-vessel disease (TVD) were compared based on age, gender, blood glucose, EF, Gensini score (GS), cTn, bundle branch block, type of myocardial infarction and percentage of the occluded vein. Results: Major percentage of the occluded vein, cTn, high-density lipoproteins, blood glucose, GS, and EF were statistically significant. The most frequent complication that occurred after acute myocardial infarction was ischemic heart failure (IHF) and it was mostly seen in RCA. Mortality, ventricular tachycardia (VT) and acute pulmonary edema (APE) were seen in acute ST-elevated MI that was close to circumflex artery (Cx) and lateral region. Cardiac tamponade or pericardial effusion was more frequent in mid-RCA. IHF, VT, atrioventricular complete block and APE were common in female patients. Male patients had only higher levels of cardiac tamponade and/or pericardial effusion (p<0.05). Mortality rate was 31.3% (n=15) in female patients and 11.6% (n=19) in male patients. Mortality and right bundle branch block were most common in acute inferoposterior myocardial infarction and IHF AIMI, as well as left bundle branch block was most common in inferolateral MI. TVD was more common in infarctions close to Cx, as well as most frequently in RCA when it was compared with the major occluded vein. Conclusion: Diagnosing an acute coronary syndrome and determining its characteristics based on RCA and the involved segment might be a predictive vision for rapidly predicting complications in TVD and mortality.

Las anomalías coronarias se presentan en 1,7% de la población general y causan el 33% de las muertes súbitas en personas jóvenes durante un esfuerzo extenuante. El uso de técnicas de imagen cardíaca ha permitido expandir el conocimiento sobre el diagnóstico de estas anomalías. Existen tres tipos de acuerdo a la clasificación de Greenberg: anomalías de origen, de curso y de terminación. Las de mayor importancia son las que causan alteraciones hemodinámicas y son la atresia coronaria, la anomalía de la arteria coronaria izquierda desde la arteria pulmonar/ anomalía de la arteria coronaria derecha desde laarteria pulmonar, el curso interarterial cuando nacen del seno coronario opuesto, las fístulas coronarias y las ectasias coronarias. La evaluación por angiotomografía coronaria multidetector permite caracterizarlas desde el ostium hasta su terminación, y para demostrar las consecuencias clínicas en el paciente existen técnicas de imagen como la resonancia magnética cardíaca de estrés, la ecocardiografía de estrés, la tomografía computarizada por emisión de fotón único y la angiografía coronaria con evaluación por flujo de reserva coronaria o radio instantáneo de onda libre que permiten definir el tratamiento.

Introduction:The anomalous origin of the right coronary artery (RCA) from the left coronary sinus coursing between the aorta and the pulmonary trunk is rare, but may cause myocardial ischemia and sudden death. Multislice CT coronaryangiography offers the possibility to visualize anomalous coronary artery origin non-invasively in details. Case Illustration: A 54-year-old man with a history of arterial hypertension, and hypercholesterolemia began to present with typical chest pain. After some non-invasive examination, he had coronary angiographythat revealed 70% stenosis at mid intermediate artery, normalLMCA, LAD and LCX. Ostium of RCA was found near the left valsava sinus afterrepeated cannulation attempts, no stenosis was found at RCA. After successful revascularization at intermediate artery, patient still had typical chest pain with positive ischemic response in treadmill test. Multislice CT coronary angiography was performedto evaluate the etiology of chest pain. The scan showed patent stent at intermediate artery and anomalous RCA origin from the left coronary sinus withacute angle take-off, luminal narrowing of the osteal-proximal part, as well as luminal compression between the ascending aorta and the pulmonary trunk (an interarterial course) while RCA appeared as dominant vessel. These features were considered as malignant coronary anomaly that could lead to the recommendation of surgical correction. Discussion: Accurate recognition and documentation of coronary artery anomalies are essential to determine the significance of such findings and to avoid furtherclinical complications. Multislice CT coronary angiography is a non-invasive imaging modality that can easily and precisely depict the origin and course of coronary artery anomalies as well as its relationship with adjacent structures. (J Kardiol Indones. 2014;35:274-7)Pendahuluan Kasus anomali dimana arteri koroner kanan berasal dari sinus koronarius kiri yang melintas diantara aorta dan arteri pulmonalis adalah jarang, namun dapat menyebabkan iskemia miokardium dan kematian jantung mendadak. Pencitraan dengan multislice CT angiografi koroner dapat mendeteksi anomali arteri koroner secara detail dan non invasif. Ilustrasi Kasus Laki laki berusia 54 tahun dengan riwayat hipertensi dan hiperkolesterolemia datang dengan keluhan nyeri dada tipikal. Setelah dilakukan beberapa pemeriksaan non invasif, pasien menjalani angiografi koroner dan didapatkan stenosis 70% pada arteri mid intermediate, normal arteri koroner cabang utama, arteri koroner anterior desendens, dan arteri koroner sirkumfleksa. Ostium arteri koroner kanan ditemukan setelah beberapa usaha kanulasi dan terletak dekat sinus valsava kiri. Tidak didapatkan stenosis pada arteri koroner kanan. Setelah dilakukan revaskularisasi pada arteri intermediate dengan sukses, pasen masih mengeluh nyeri dada tipikal dengan respon iskemik positif pada uji latih jantung.
