Evaluation of Acute Physiology and Chronic Health Evaluation III predictions of hospital mortality in an independent database

Abstract: APACHE III accurately predicted aggregate hospital mortality in an independent sample of U.S. ICU admissions. Further improvements in calibration can be achieved by more precise disease labeling, improved acquisition and weighting of neurologic abnormalities, adjustments that reflect changes in treatment outcomes over time, and a larger national database.

“…Mortality outside the ICU was regrettably much higher, up to 53% (28). This has been confirmed by other reports (29)(30)(31)(32)(33)(34)(35), and it strongly suggests that all patients with severe sepsis and septic shock should be admitted to the ICU.…”

“…The APACHE(Acute Physiology, Age, Chronic Health Evaluation) scoring system, a prototype of a classification system for disease severity, has continued to evolve into newer versions over the last twenty years through validations of discrimination and calibration regarding prognosis (4)(5)(6). The APACHE III scoring system as a methodology of risk prediction of hospital mortality (6) is currently used in many studies worldwide (10)(11)(12)(13).…”
