The purpose of this study is to document the improvement observed in two cases of Parkinson's disease (PD) after dental treatment. The first subject is a man in his 60s with severe Parkinson's disease; medication has not been very effective in this case. Prior to treatment, he was unable to stand without support due to rigidity. Just after removing as much of the dental infection as possible, he was able to walk, albeit slowly, and as a result of continuing treatment, one month later, the symptoms had significantly improved. The second subject is a woman in her 40s, who became aware of joint stiffness seven years ago, and was later diagnosed with PD independently at three hospitals. Her main symptoms were rigidity, knee pain, and speech disorder. The dopamine medication worked well against rigidity, but the symptoms reappeared after the medication stopped working. Her condition was significantly improved just after one tooth with an apical lesion was extracted. Although the underlying mechanism has not been clarified, I hypothesize that, at least in these cases, negative signals that passed through the trigeminal nerve to the midbrain affected predominantly the dopaminergic neurons in the substantia nigra of the midbrain. Removal of the harmful signals from the oral area resulted in normalization of the substantia nigra. Further research should be promoted with dental and medical cooperation.

Treatment of Elderly Bedridden Patients with Removable Dentures to Promote Their Daily Physical Activities: A Case-Control Study

Bedridden patients remain in bed for various reasons, such as chronic illness, old age, and disability, and they cannot perform self-care activities completely or partially. The provision of care for bedridden patients is a major issue in the aging population. Effective rehabilitation is associated with several factors such as cooperation between the patient and the medical staff, selection of effective facilities, and the financial burden. The objective of this study was to evaluate the effect on the activities of daily living (ADL) of occlusion restoration using removable dentures in bedridden older people. This case-control study comprised 32 bedridden older patients who were divided into two groups as follows: denture (n = 18) and no-denture (n = 14). No rehabilitation was provided to any of the patients. The patients were evaluated 3 months after placing the denture, and the ADLs were compared between the two groups. Of the 18 bedridden patients who received removable dentures, 55.6% demonstrated improvements in the ADLs, and the effects persisted for at least 3 months. No improvement in the ADL was observed in the no-denture group. The results of this study suggested that adequate occlusal support can improve the ADL of the elderly. Thus, it is important to take care of oral health early in life in order to maintain oral health, which could help prevent the decrease in the ADL during the later stages of life.

Occlusion and mandibular positions substantially affect full-body functioning and performance in sports. This report illustrates the case of a 27-year-old female long-distance runner who had intermittent lower back and hip pain. Since her bilateral maxillary first molars were missing, small partial dentures were placed in the position of those missing molars in order to improve occlusion. Even right after the right-side denture was placed, her performance on the straight leg raising test and the strength of the iliopsoas muscle in the supine position improved. Her performance improved further after the left-side denture was placed. This report shows that occlusal restoration causes a definite improvement in body function, even in cases of small missing teeth.
