Cancer-related fatigue (CRF) has deleterious effects on physical, social, cognitive, and vocational functioning, and causes emotional and spiritual distress for patients and their families; however, it remains under-recognized and undertreated. This article critically reviews and integrates the available empirical evidence supporting the efficacy of pharmacologic and nonpharmacologic treatment approaches to CRF, highlighting new evidence since 2007 and 2009 Putting Evidence Into Practice publications. Interventions that are recommended for practice or likely to be effective in improving fatigue outcomes include exercise; screening for treatable risk factors; management of concurrent symptoms; yoga; structured rehabilitation; Wisconsin ginseng; cognitive-behavioral therapies for insomnia, pain, and depression; mindfulness-based stress reduction; and psychoeducational interventions such as anticipatory guidance, psychosocial support, and energy conservation and activity management. This information can be applied to improve the management of CRF, inform health policy and program development, shape the design of clinical trials of new therapies for CRF, and drive basic and translational research.

StreszczenieWprowadzenie: Zmęczenie jest objawem najczęściej zgłaszanym przez pacjentów w trakcie leczenia onkologicznego. Brief Fatigue Inventory (BFI; Mendoza, Wang, Cleeland, Morrissey, Johnson, Wendt, Huber, 1999) jest krótkim i kompleksowym narzędziem do oceny nasilenia zmęczenia i jego wpływu na funkcjonowanie wśród chorych na nowotwory. Cel pracy: Celem badania była ocena właściwości psychometrycznych polskiej wersji BFI, w tym ustalenie struktury narzędzia oraz jego zgodności wewnętrz-nej w grupie pacjentów onkologicznych. Materiał i metody: W badaniu uczestniczyło 200 hospitalizowanych pacjentów (w tym 55% mężczyzn) o średniej wieku 63 lata (SD = 10,29) -66 pacjentów w trakcie ustalania rozpoznania, 70 w trakcie leczenia pierwotnego i 64 w trakcie leczenia z powodu wznowy lub rozsiewu raka płuca. Wyniki i wnioski: Trafność teoretyczną BFI zweryfikowano za pomocą eksploracyjnej analizy czynnikowej (EFA), która potwierdziła jednoczynnikowy charakter narzędzia. Uzyskano wysokie ładunki czynnikowe (nie mniejsze niż 0,73), co przełożyło się na wysokie współczynniki rzetelności, zarówno dla wyniku ogólnego BFI, jak i jego podskal -nasilenia znużenia i jego wpływu na rozmaite obszary funkcjonowania (α Cronbacha > 0,90). Porównanie zmęczenia pomiędzy grupami pacjentów wykazało istotne statystycznie różnice w odniesieniu do jego wpływu na rozmaite obszary funkcjonowania, aczkolwiek wielkość efektu (η 2 ) nie przekraczała 5%. Analiza przeprowadzona testem post-hoc potwierdziła gorsze funkcjonowanie pacjentów w trakcie leczenia z powodu wznowy lub rozsiewu raka płuca niż pacjentów w trakcie ustalania rozpoznania. Na podstawie uzyskanych wyników można stwierdzić, że BFI jest rzetelnym narzędziem do oceny zmęczenia wśród chorych na raka płuca, a w trakcie leczenia z powodu wznowy lub rozsiewu nowotworu może odzwierciedlać ograniczenia w ich codziennym funkcjonowaniu. AbstractIntroduction: Fatigue is one of the most common symptom reported by cancer patients. The Brief Fatigue Inventory (BFI; Mendoza, Wang, Cleeland, Morrissey, Johnson, Wendt, Huber, 1999) is a short and comprehensive instrument to assess severity of fatigue and fatigue-related impairment in cancer patients. Aim: The aim of the study was to evaluate the psychometric properties of the Polish version of the BFI, and to determine the structure of the tool and its reliability in a group of cancer patients.
