Evaluation of a Combination of Self-Administered Fluoride Procedures for Control of Dental Caries in a Non-Fluoride Area: Findings after 2 Years

Abstract: In 1972, a self-administered fluoride program was initiated in Nelson County, Va., a fluoride-deficient area. Children in elementary school (grades 1–6) ingest daily an acidulated phosphate-fluoride tablet (1 mg F), rinse weekly with a 0.2% NaF solution and are provided with an F dentifrice for use at home. Baseline dental exams (DMFS) were made of 2,135 children in grades 1–12. In 1974, follow-up exams were made in grades 1–8 (ages 6–13). Baseline findings for children aged 6–13 in 1972 showed a mean prevalen… Show more

“…Because school officials believed that it was too difficult to conduct mouthrinsing at the senior high level, these students (ages 14-17) practiced only the daily fluoride tablet regimen at school and brushed their teeth ad libitum with a fiuoride-containing dentifrice at home. Details of study methods and findings for the combined fiuoride regimen have been published elsewhere (1)(2)(3)(4)(5).…”

“…In 1983, after 11 yr of a school-based fiuoride program in Nelson County, Virginia, the overall prevalence of dental caries was 65"/o lower than in 1972 (1). As demonstrated at each follow-up examination (2)(3)(4)(5), caries reductions were greater on smooth (mesiodistal) than on pit and fissure surfaces (occlusal and buccolingual). Also in 1983, approximal caries constituted only 8' !4i of the total decayed, missing, and filled surfaces (DMES), whereas caries on pit and fissure surfaces accounted for 92% of the remaining dental decay among the participants.…”

“…Dietary fluoride supplements were part of a comprehensive fluoride regimen provided to schoolchildren, 5-17 years of age in Nelson County, Virginia, from 1972-1983 (19)(20)(21)(22)(23). In addition to using a fluoride tablet daily in school, children were given fluoride toothpaste for use at home and participants in elementary and junior high school (essentially ages 5-14) also rinsed once a week in school with a 0.2 percent sodium fluoride solution.…”

“…Public health administrators must seek to maximize caries reduction, while minimizing dental fluorosis. [4] According to some reports, [5][6][7] the caries prevalence tends to reduce with increasing fluoride levels in the drinking water, thus indicating a negative association between fluoride and dental caries. However, some studies have indicated no relation [8] or a positive relation [9,10] between the fluoride concentration in the drinking water and dental caries.…”
