Evaluation and validation of social and psychological markers in randomised trials of complex interventions in mental health: a methodological research programme

Abstract: BackgroundThe development of the capability and capacity to evaluate the outcomes of trials of complex interventions is a key priority of the National Institute for Health Research (NIHR) and the Medical Research Council (MRC). The evaluation of complex treatment programmes for mental illness (e.g. cognitive–behavioural therapy for depression or psychosis) not only is a vital component of this research in its own right but also provides a well-established model for the evaluation of complex interventions in ot… Show more

“…As the evidence base for low-intensity interventions in OCD grows, appropriate use of techniques such as metaregression 98 and mediational analysis 99 may provide useful insights into key 'active ingredients' that might enhance their effectiveness. Equally, better understanding of the sorts of patient groups most likely to benefit 100 may enable better targeting of interventions as part of a combined stepped-and stratified-care model.…”

“…If the treatment effect on the primary outcome (HFNS problem rating) is observed to be statistically significant, exploratory analysis will be conducted to explore the efficacy mechanism in relation to several putative mediators (such as HFNS beliefs, mood, HFNS frequency) using mediation analysis performed by the product-of coefficients approach [45]. If there is not effect on the primary outcome and the estimated effect size is less than .4, explanatory analysis will be conducted in order to determine whether poor adherence to the intervention or centre level variation were potentially related to the null finding.…”

“…The end of therapy assessment was at 9 months. Second, a latent class subgroup analysis (Dunn et al, 2015) and principal stratification (Frangakis and Rubin, 2002) was used to investigate the effect of each treatment component (such as case formulation, cognitive techniques or behavioural strategies etc.) on outcome.…”
