“…Longitudinally extensive transverse myelitis (LETM) is a magnetic resonance imaging diagnosis defined as intramedullary hyperintense T2-weighted signal abnormality spanning three or more vertebral segments (Wingerchuk et al 2006). The differential diagnosis of LETM is broad, including compressive, infective, demyelinating/inflammatory, vascular and neoplastic causes (Mirbagheri et al 2016), although use of this terminology commonly implies a demyelinating/inflammatory aetiology. We now report a paediatric patient from the LHON Qld1 family with extensive myelopathy resembling LETM.…”

“…The initial clinical response to treatment may cause development of new lesions or expansion of preexisting ones due to lysis and release of tubercular proteins. [ 9 ] In our patient, LETM was detected before MTB treatment. We consider that in our case, the LETM mechanism is that MTB shares antigens with myelin basic protein, thus sensitizing lymphocytes to mycobacterium and facilitating their attachment to myelin.…”

“…LETM has a broad list of differential diagnoses, ranging from multiple sclerosis to spinal cord infarction. LETM is known to have a hyperacute, acute, subacute, or chronic onset [11] . LETM with a subacute onset is seen in subacute degeneration of the spinal cord as mentioned above.…”
