Evaluating the Utility of the Lateral Elbow Radiograph in Central Articular Olecranon Reduction: An Anatomic and Radiographic Study

Abstract: Intra-articular olecranon malreductions are inconsistently recognized by trauma surgeons on the lateral elbow radiograph. Therefore, articular incongruity may still be present after surgical fixation of comminuted olecranon fractures. We must further define the radiographic anatomic representation of the articular olecranon to improve surgical reduction and clinical outcomes.

“…In other reports, inflammatory and immunemediated etiology has been suggested as the cause of primary DJD in cats (14), and immune dysfunction has been seen in association with DJD in a genomic/proteomic study (15). Some studies have reported the pathology of feline arthritis (16,17), and it is gradually being elucidated. To further our understanding of the pathophysiology of feline DJD, it is necessary to prove these relationships precisely (18).…”

“…In general, orthogonal radiography has been recommended for use when diagnosing orthopedic lesions, and only appendicular and vertebral joints were assessed by orthogonal radiography in the present study. Although evaluation of the precise internal joint structure is difficult based on a single-plane radiograph (17), the utility of the single radiograph for evaluation of joint incongruity has been reported (29) in a human study. Appendicular OA, especially the hip and stifle joints, probably requires two radiographic views for diagnosis.…”

“…In addition, the quality of fracture reduction was evaluated with plain radiographs which may not as accurate as the CT scan. 20,35 Although some of the data of suture fixation in comminuted olecranon fracture were published previously, 12,17 our study presented the different technique in fixation of the articular portion and outer surface. Furthermore, this study is the first to focus on quality of reduction from suture fixation in olecranon fracture.…”

“…For example, malreduction was previously identified on lateral elbow radiographs only 73% of the time, with interrater reliability of 0.28 to 0.66. 22 Furthermore, 86% of fractures categorized as simple on lateral radiograph were identified as comminuted on CT. 6 Inconsistency in preoperative radiograph quality can complicate this issue, especially when patients are splinted or otherwise immobilized. CT would likely increase sensitivity and agreement in identifying articular impaction.…”
