Evaluating the Role of mpMRI in Prostate Cancer Assessment

Abstract: Prostate cancer is the most common malignancy among American men. The role of multi-parametric MRI has recently gained more importance in detection of prostate cancer, its targeted biopsy, and focal therapy guidance. In this review, uses of multi-parametric MRI in prostate cancer assessment and treatment are discussed.

“…Diffusion-weighted magnetic resonance imaging (DWI) is a noninvasive, radiation-free tool that does not require the use of exogenous contrast agents, is accessible, and is easy to incorporate into the clinical routine. Over the last decade, DWI has demonstrated good diagnostic performance in multifarious tumors of various etiologies (10)(11)(12)(13)(14) . The aim of this study was to determine the performance of DWI in distinguishing between malignant and benign thoracic lymph nodes.…”

“…One of the main issues is the difficulty to discriminate between healthy, benign, and malignant prostatic tissues. Furthermore, other conditions often associated with the disease (eg, prostatitis, atrophy, or hemorrhages) may affect the signal intensity, therefore compromising the overall reliability of the diagnosis . Although alternative MRI‐based diagnostic tools were already proposed, a possible way to overcome these limitations could be to support the readout from T 2w images with information achievable from T 1w images acquired after the injection of PCa‐specific molecular MRI agents.…”

Indeed, the use of efficient targeted probes that can specifically reach the tumor could directly and uniquely image the tumor tissue.…”
