To review the indications and efficacy of Intense Pulsed Light (IPL) application in the treatment of Meibomian Gland Dysfunction (MGD). Its main purpose is to describe its physiology, efficacy, indications, and adverse effects. Patients and Methods: A two database (PubMed, EMBASE) search was performed from July 2017 to July 2022 using the MeSH terms ("Intense Pulsed Light" AND ("Meibomian Gland Dysfunction" OR "Dry Eye"). We included randomized studies and systematic reviews with meta-analysis. Exclusion criteria were non-randomized trials, studies enrolling non-MGD dry eye disease, and other works older than 5 years. Results: Current literature shows that IPL is an effective and safe treatment modality for severe dry eye. Available evidence shows improvement of symptoms and objective indicators, such as noninvasive breakup time, thickness of lipid layer, and Schirmer test. However, our review concluded that the beneficial effects of IPL may lose some efficacy at 6-months after the initial session, and subsequent sessions may be required. Thus, IPL treatment should not be considered as first-line therapy for MGD but instead as an adjuvant option to the standard of care. The optimal treatment modality remains unknown and should be tailored according to each patient's phenotype, clinician's experience, and available technology. There is evidence that IPL treatment may down-regulate proinflammatory markers (such as interleukin (IL) 6, IL17a, IL-1) and Prostaglandin E2 (PGE2). Conclusion: MGD is a multifactorial disease and IPL treatment seems a promising treatment modality. Despite this, more evidence is needed to study its benefits -since this is an emerging technology, it is expected an increase in comparative studies in the following years, with longer follow-up periods, which may enable more precise conclusions about this treatment modality.

ObjectivesTo assess the safety, efficacy, patients’ satisfaction and acceptability of Rexon-Eye electrotherapy in treating Asian severe dry eye disease (DED) patients.MethodsProspective parallel-arm pilot study recruiting 40 DED Chinese patients with >moderate recalcitrant DED (Contact Lens Research Unit [CCLRU] > grade 2). Subjects were randomized into 2 groups, undergoing four weekly treatment sessions each: group 1 received full treatment power; group 2 received control treatment (power 1 treatment). Non-invasive tear break-up time (NIBUT), cornea fluorescein staining graded via CCLRU and Schirmer’s I test were compared pre- and 2 months post-treatment. The SPEED and QUEST questionnaires that evaluated subjective symptoms and treatment satisfaction, respectively, at baseline and 2 weeks post-treatment were carried out. Tear cytokine levels in both groups were examined at 2 weeks post-treatment.ResultsThe amount of improvement in post-treatment corneal staining in the inferior corneal zone was significant in Group 1 (p = 0.038) but not in Group 2 (p = 0.832). Group 1 eyes with worse baseline staining (total score >9.8) had a significantly greater reduction of corneal staining than those with better baseline staining (−11.7 ± 1.98 vs. −4.6 ± 2.89, p < 0.001). There were no other significant differences in NIBUT, Schirmer’s 1 and cornea fluorescein staining grading within or between the groups.: Group 1 (n = 24) had improved subjective dryness scores compared to Group 2 (n = 16) (SPEED score: 6.38 + 4.16 vs. 10.0 + 6.36, p = 0.04). No significant differences were seen in 11 tear cytokine levels at 2 weeks post-treatment between the 2 groups.ConclusionIn Asian DED patients treated with Rexon-Eye, inferior cornea staining showed significant improvement compared to placebo, and eyes with greater cornea staining at baseline achieved a greater improvement in staining. There were no other significant improvements in NIBUT and Schirmer’s 1. Rexon-Eye also improved subjective DED scores in 41.7% of eyes without any adverse effects.

We suggest that quantum molecular resonance (QMR) may have therapeutic effects for managing mild acne.A 37-year-old woman and a 39-year-old woman presented with erythematous papules and fine wrinkles on their face. They were otherwise healthy. Physical examination revealed papules on their cheeks and perinasal area. Three experienced dermatologists examined the lesions and individually arrived at the diagnosis of acne vulgaris. According to the Global Acne Grading System (GAGS), 1 the first patient had mild acne with a score of 10 (Figure 1A). The second patient scored 12 on the GAGS, with only papules (Figure 2A). Punch biopsies were not performed. The patients refused oral and topical treatment. The patients aimed for fine wrinkle improvement along with acne treatment and agreed to treatment with QMR. All procedures were conducted after obtaining informed consent and clinical approval. QMR (Corage 2.0, QuanteQ, Korea) with an energy level of 35 W per 100 s was performed at three sessions at 3-week intervals. The patients maintained their habits without changes.After the third visit, the first patient's acne lesions cleared (GAGS score 0) and she was satisfied with no side effects (Figure 1B). Supplementarily, UV fluorescence photography (Mark Vu, PSI Plus, Korea) was taken. Fluorescent spots highlight porphyrin. Compared to baseline (Figure 1C), diminished spots were noted after treatment (Figure 1D).
