Evaluating Nurses' Implementation of an Infant-Feeding Counseling Protocol for HIV-Infected Mothers: The Ban Study in Lilongwe, Malawi

Abstract: A process evaluation of nurses' implementation of an infant-feeding counseling protocol was conducted for the Breastfeeding, Antiretroviral and Nutrition (BAN) Study, a prevention of motherto-child transmission of HIV clinical trial in Lilongwe, Malawi. Six trained nurses counseled HIVinfected mothers to exclusively breastfeed for 24 weeks postpartum and to stop breastfeeding within an additional four weeks. Implementation data were collected via direct observations of 123 infant feeding counseling sessions (3… Show more

“…Compared with 2 other groups of Malawian infants in Lungwena with short periods of exclusive breastfeeding and ,20% prevalence of HIV among mothers, on average, the BAN infants were approximately the same weight but ;1-3 cm longer at 6 mo of age (49,50). Possible explanations for the somewhat better linear growth of BAN infants are residence in an urban area compared with the rural Lungwena cohorts and the extensive counseling and support for exclusive breastfeeding provided by the BAN study nurses (33), which was not available in Lungwena.…”

“…All women were intensively counseled to exclusively breastfeed their infants until 24 wk of age and to wean rapidly by 28 wk (33). Although it is possible that there are delayed effects of maternal supplementation on infant growth, only data up to 24 wk are included in the present analysis, because the infants began receiving LNS for their own consumption at that time.…”

“…In addition, prevalence of any diarrhea in BAN infants was lower for similar time periods than among Zambian HIV-exposed infants who were weaned at 4 mo (43% at 7-9 mo and 44% at 10-12 mo) as well as those who were still breastfed (38% at 7-9 mo and 39% at 10-12 mo) (5). This could be attributable in part to the provision of LNS (which is resistant to bacteriologic growth) as a breastmilk replacement or to the counseling on replacement feeding practices provided by the study (19,42).…”

“…The randomized interventions were provided from 0 to 28 wk. Mothers in the BAN Study were counseled to exclusively breastfeed for 24 wk then wean their infants within 4 wk (19). To address the possibility of inadequate feeding following weaning (20), all infants were provided with lipid-based nutrient supplements (LNS) from 24 to 48 wk of age.…”

“…This analysis includes data from 0-24 weeks, when mothers were provided with intensive counseling to exclusively breastfeed. 26,27 Maternal Hb was measured at prenatal screening and maternal and infant Hb (g/L) was measured at birth, 2, 6, 12, 18 and 24 weeks postpartum from venous blood samples with a Beckman Coulter AcT or AcT 5-part Differential Analyzer (Beckman Coulter, Fullerton, CA). Infant HIV status was tested with Amplicor 1.5 DNA polymerase-chain reaction.…”
