BackgroundThe level of traditional medicine use, particularly Jamu use, in Indonesia is substantial. Indonesians do not always seek timely treatment for malaria and may seek self-medication via traditional medicine. This paper reports findings from the first focused analyses of traditional medicine use for malaria in Indonesia and the first such analyses worldwide to draw upon a large sample of respondents across high-risk malaria endemic areas.MethodsA sub-study of the Indonesia Basic Health Research/Riskesdas Study 2010 focused on 12,226 adults aged 15 years and above residing in high-risk malaria-endemic provinces. Logistic regression was undertaken to determine the significant associations for traditional medicine use for malaria symptoms.FindingsApproximately one in five respondents use traditional medicine for malaria symptoms and the vast majority experiencing multiple episodes of malaria use traditional medicine alongside free antimalarial drug treatments. Respondents consuming traditional medicine for general health/common illness purposes every day (odds ratio: 3.75, 95% Confidence Interval: 2.93 4.79), those without a hospital in local vicinity (odds ratio: 1.31, 95% Confidence Interval: 1.10 1.57), and those living in poorer quality housing, were more likely to use traditional medicine for malaria symptoms.ConclusionA substantial percentage of those with malaria symptoms utilize traditional medicine for treating their malaria symptoms. In order to promote safe and effective malaria treatment, all providing malaria care in Indonesia need to enquire with their patients about possible traditional medicine use.

Herbal medicine has been used empirically worldwide including in Indonesia but is not recorded properly yet. This analysis aimed to obtain the herbal medicine service patterns especially the combination of treatments, amount and types of herbal preparation. Jamu Registry was an observational study on electronic medical recording of herbal medicine services by medical doctor on 10 health problems in several cities of Indonesia. A total of 54 medical doctors participated in Jamu Registry 2016 and 2018 recorded 1660 visits of 565 patients aged 10 to 85 years old (mean 49,8), treated with herbal medicine. More than half patients (68.1%) were diagnosed with single diagnosis, 24.4% with one comorbidity, and 7.4% with two comorbidities. Most patients (70.4%) were treated with herbal medicine only, while others combined with conventional medicine (22.0%), other traditional modalities (4.6%), conventional medicine and other traditional medicine (3.0%). Numbers of herbal medicine prescribed were varied from one to five. The herbal preparation types were extracts in capsule (38.6%), infusion (31.5%), ground dried herbs in capsule (25.8%), and the others were decoction, syrup, fermentation liquid, ground dried herbs, or brewed herbs. The top 5 diagnoses recorded were dyspepsia, diabetes mellitus, hypertension, hemorrhoid, and arthritis. Almost three quarters patients had no comorbidity; most patients were treated with herbal only, mostly in form of capsule and infusion. Herbal medicine services need to be recorded continuously to enrich the traditional medicine health services data.

The utilization of traditional health services and the use of traditional medicine in Indonesia is still high. There are socio-cultural-natural resources connection in the use of traditional health services and traditional medicine. This study examines Basic Health Research (Riskesdas) 2018 data relating to Indonesia's top ten provinces' relative position, whose community exercises self-traditional health practices and utilizing traditional health services. The analysis was conducted by using PCA-Biplots. Results showed similarities between North Maluku-Maluku-West Papua; Central Sulawesi-South Sulawesi-East Nusa Tenggara-Papua; Special Region of Yogyakarta-Central Java-East Java; South Kalimantan-Banten, while the others were scattered. The utilization of TOGA had a positive correlation with the utilization of traditional medicines. The result of variable diversity identification showed that the community utilizes traditional health services (83.29%) was higher than community exercising self-traditional health practices (73.19%). Actively monitoring, improving information sharing, and educating people on traditional medicine applications, particularly non-communicable disease issues, should be done according to traditional medicine variables' main characteristics in the region. Traditional medicine should serve promotive and preventive health initiatives, as its efficacy in therapeutic use is still debatable. Abstrak Pemanfaatan pelayanan Kesehatan tradisional (yankestrad) dan penggunaan obat tradisional masih cukup banyak. Terdapat keterkaitan sosial, budaya, dan sumber daya alam dalam pemanfaatan yankestrad dan penggunaan pengobatan tradisional lokal. Penelitian ini menganalisis posisi relatif 10 besar provinsi di Indonesia yang melakukan upaya kestrad sendiri dan memanfaatkan yankestrad berdasarkan data Riskesdas 2018. Analisis posisi relatif dalam artikel ini adalah PCA-Biplot. Hasil analisis menunjukkan pola pengelompokan kemiripan sebagai berikut: Malut-Maluku-Pabar; Sulteng-Sulsel-NTT-Papua; DIY-Jateng-Jatim; Kalsel-Banten, dan lainnya tersebar. Variabel pemanfaatan TOGA, semakin positif variabel, maka diikuti oleh pemanfaatan obat tradisional yang semakin baik. Hasil identifikasi keragaman variabel pada pengelompokan 10 besar provinsi dengan masyarakat memanfaatkan yankestrad (83,29%) mempunyai nilai lebih tinggi daripada masyarakat melakukan upaya kestrad sendiri (73,19%). Pemerintah melalui dinas terkait harus melakukan pemantauan, pemberian informasi dan edukasi pengobatan tradisional khususnya untuk penyakit tidak menular dengan penyesuaian terhadap karakteristik pemanfaatan pengobatan tradisional di wilayah tersebut.
