EuroSCORE II

Abstract: Cardiac surgical mortality has significantly reduced in the last 15 years despite older and sicker patients. EuroSCORE II is better calibrated than the original model yet preserves powerful discrimination. It is proposed for the future assessment of cardiac surgical risk.

“…Specifically, a history of HF is not included in commonly used cardiac surgery mortality risk scores. The EuroScore II35 incorporates preoperative EF but does not account for HF in the calculation of early operative mortality. The Society of Thoracic Surgeons risk model36 includes preoperative EF and New York Heart Association functional status (a classification of physical activity limitations) but not a history of HF.…”

“…So far, there are virtually no prospective randomized trials comparing minimally invasive valve surgery (MIMVS) and standard sternotomy (ST) in an unbiased way, and the majority of evidence rely on observational studies. Moreover, definition of the level of risk has been a controversial issue, mainly because the currently available tools of risk prediction in cardiac surgery such as EuroSCORE II [3] and The Society of Thoracic Surgery Predicted of Risk of Mortality [4] lose calibration and discrimination in the upper boundaries or deciles of surgical risk. Hence, defining where to draw the "red line" above which patients should be considered at high risk remains a challenge and an issue of debate.…”
