European Surveillance of Sexually Transmitted Infections (ESSTI): the first combined antimicrobial susceptibility data for Neisseria gonorrhoeae in Western Europe

Abstract: These are the first sentinel surveillance data for Western Europe for N. gonorrhoeae and they have implications for choice of antimicrobial for treatment of gonorrhoea on a European and a local level. This is the start of the formation of a European gonococcal antimicrobial surveillance programme (EURO-GASP).

“…A 100% of the isolates were susceptible to ceftriaxone, but the 7.7% showed reduced susceptibility (MIC: 0.125 to 0.25 µg / ml) and did not express extended spectrum beta lactamase ( LEE) (García, 2010b) This situation is similar to other countries, in Australia (2005) 1% of the isolates of N. gonorrhoeae had MIC ≥ 0.062 µg / ml; 12.1% in Amsterdam in 2008;3% in Russia (2004) and elsewhere (Tanaka, 2006;Barry, 2009;Martin, 2006).…”

“…Ceftriaxone (third-generation cephalosporin, parenteral) retains its activity against N. gonorrhoeae, but recently have been appearing isolates with decreased sensitivity in various regions of the world (Martin, 2006;Barry, 2009, Tapsall, 2009Chisholm, 2010;Deguchi, 2010;Garcia, 2010a).…”

“…Presently, the recommended first-line treatments for gonorrhea in most countries include antibiotics such as cefixime, ceftriaxone, spectinomycin, and in some cases azithromycin or ciprofloxacin. However, resistance to fluoroquinolones and azithromycin is rapidly increasing [8,10]. The use of the quinolone group of antibiotics for the treatment of gonorrhoea has been discontinued in India for quite some time because of reported high levels of resistance.…”

“…This re-emergence of gonorrhoea was primarily due to outbreaks among MSM, increased transmission among young heterosexuals of both sexes and resistance to antimicrobials like penicillin, tetracycline, fluoroquinolones used in the traditional treatment of the gonococcal infection. 11,20 In addition adoption of more risky sexual behaviour for e.g. an increase in the number of sexual partners and the number of new/ casual sexual partners and / or low use of condoms in the Swedish population were also reported as the most likely reason for this recent increase in gonorrhoea cases.…”
