European guidelines for topical photodynamic therapy part 1: treatment delivery and current indications – actinic keratoses, Bowen’s disease, basal cell carcinoma

Abstract: Topical photodynamic therapy (PDT) is a widely used non-invasive treatment for certain non-melanoma skin cancers, permitting treatment of large and multiple lesions with excellent cosmesis. High efficacy is demonstrated for PDT using standardized protocols in non-hyperkeratotic actinic keratoses, Bowen's disease, superficial basal cell carcinomas (BCC) and in certain thin nodular BCC, with superiority of cosmetic outcome over conventional therapies.Recurrence rates following PDT are typically equivalent to exi… Show more

“…Although c‐PDT is a successful treatment for AK,5, 9, 24 the unpredictability of tolerance during treatment in some patients, extended treatment times, and costly instrumentation has limited its use. Daylight PDT is a simple and practical new treatment option for AK that allows large areas of actinic damage to be treated with reduced in‐clinic treatment times and little or no pain.…”

“…Protoporphyrin IX (PpIX)-induced PDT (PpIX-PDT) is utilised as topical method of localised ablation of certain non-melanoma skin cancers and precancers [4]. 5-Aminolevulinic acid (ALA)-induced photodynamic therapy (ALA-PDT) was first introduced experimentally by…”

“…PDT has been also used for treatment of Bowen's disease, or squamous cell carcinoma in situ and is recommended for both extensive involvement and poor healing sites [98]. The approved MAL-PDT dosing regimen for Europe consists of two treatments 7 days apart, repeated at 3 months, if needed [99].…”
