Background The use of artemisinin as a monotherapy resulted in the emergence of artemisinin resistance in 2005 in Southeast Asia. Monitoring of artemisinin combination therapy (ACT) is critical in order to detect and prevent the spread of resistance in endemic areas. Ex vivo studies and genotyping of molecular markers of resistance can be used as part of this routine monitoring strategy. One gene that has been associated in some ACT partner drug resistance is the Plasmodium falciparum multidrug resistance protein 1 (pfmdr1) gene. The purpose of this study was to assess the drug susceptibility of P. falciparum populations from Thiès, Senegal by ex vivo assay and typing molecular markers of resistance to drug components of ACT currently used for treatment.Methods The ex vivo susceptibility of 170 P. falciparum isolates to chloroquine, amodiaquine, lumefantrine, artesunate, and artemether was determined using the DAPI ex vivo assay. The high resolution melting technique was used to genotype the pfmdr1 gene at codons 86, 184 and 1246.ResultsA significant decrease in IC50 values was observed between 2012 and 2013: from 13.84 to 6.484 for amodiaquine, 173.4 to 113.2 for lumefantrine, and 39.72 to 18.29 for chloroquine, respectively. Increase of the wild haplotype NYD and the decrease of the mutant haplotype NFD (79 and 62.26 %) was also observed. A correlation was observed between the wild type allele Y184 in pfmdr1 and higher IC50 for all drugs, except amodiaquine.ConclusionThis study has shown an increase in sensitivity over the span of two transmission seasons, marked by an increase in the WT alleles at pfmdr1. Continuous the monitoring of the ACT used for treatment of uncomplicated malaria will be helpful.

Malaria is a parasitic disease defined as a major problem of public health in Côte d'Ivoire. One of the constraints of the control of this disease might be the genetic diversity of the main pathogen, Plasmodium falciparum. This study aims to evaluate the genetic diversity of Plasmodium falciparum in individuals at the regional health center in Daloa, Côte d'Ivoire. A total of 380 blood samples were collected from patients, from June to October, 2015, based on malaria clinical signs and symptoms. Whole DNA was extracted from blood samples tested positive by microscopic examination for P. falciparum. A nested PCR targeting the msp-1(block 2) allelic families was implemented for the parasite genetic diversity. The associations between malaria infection and socio-environment factors were estimated. Out of the 380 examined patients, 262 were tested positive by microscopic examination representing a prevalence of 68.95%. Genotyping from 160 sample randomly chosen allowed to identify 35 alleles comprising 13 K1, 12 Mad 20 and 10 Ro33 type. Mixed infections were significantly correlated with the parasite load. The number of parasite strains hosted by a patient was significantly linked to the level of anemia and the parasite density. The multiplicity of the infection (MOI) decreased significantly with the age. Genetic variability of strains is a very important parameter to be taken into account in the epidemiology of malaria. The present results should draw the attention of decision-makers to develop a better monitoring strategy for the elimination of malaria in an endemic country such as Côte d'Ivoire.

Le paludisme est une maladie endémique en Afrique intertropicale. En vue d'évaluer les différents efforts consentis dans la lutte contre le paludisme dans le monde, l'Organisation Mondiale de la Santé (OMS) a présenté en 2016 la tendance de l'évolution de la maladie dans différents pays, grâce aux données épidémiologiques existantes. Certaines régions du Tchad et surtout Abéché n'ont pas bénéficié de cette évaluation à cause des données insuffisantes. L'objectif général de cette étude était de déterminer la prévalence du paludisme chez les patients de l'Hôpital Provincial d'Abéché (HPA) et de spécifier les facteurs des risques associés à cette pathologie endémique. Nous avons mené une étude descriptive transversale de Mai à Juin 2017 à l'HPA sur 350 patients à qui un examen de Goutte Epaisse et la Recherche d'Hématozoaire (GERH) ont été réalisés. Les patients ont été soumis à un entretien sur la base d'un questionnaire basé sur la proximité de l'eau, l'usage des moustiquaires, absence de désinsectisation. Les données collectées ont été analysées et les résultats révèlent que la prévalence du paludisme était de 36,57% et était significativement associée à la non utilisation de moustiquaire imprégnée d'insecticide (p<0,001), au manque de connaissances sur le paludisme (p= 0,01) et au fait d'habiter à proximité des eaux stagnantes (p= 0,023), ce qui favoriserait la forte prévalence de cette pathologie. Ces données peuvent être utilisées pour la prise de décision dans les programmes de lutte antipaludique.
