Background: Gastric cancer is the second most common gastrointestinal cancer and is more common in the East, compared to the West. This study assesses the trend of gastric cancers in Brunei Darussalam, a developing nation with a predominantly Malay population. Materials and Methods: The cancer registry from 1986 to 2012 maintained by the Department of Pathology, the only State Laboratory at the RIPAS Hospital, Ministry of Health, was reviewed and data extracted for analyses. The age standardised rate (ASR) and age specific incidence rate were calculated based on the projected population. Cancers diagnosed below 45 years were categorised as young gastric cancer. Results: Over the study period, there were a total of 551 cases of gastric cancer diagnosed. The most common type was adenocarcinoma (87.9%), followed by lymphoma (6.1%) and gastrointestinal stromal tumour (2.8%). The overall mean age at diagnosis was 61.9 years old (range 15 to 98) with an increasing trend observed, but this was not significant (ANOVA). There were differences in the mean age at diagnosis for the different races (p=0.003 for trend), but not the gender (p=0.105). Young gastric cancer accounted for 14.9%, being more common in women, and in Expatriate and Malay populations compared to the Chinese. There was a decrease in the ASR, from 17.3/100,000 in 1986-1990 to 12.5/100,000 in 2006-2010. Chinese had a higher overall ASR (20.2/100,000) compared to the Malays (11.8/100,000). The age specific rates were comparable between men and women until the age group 55-59 years when the rates started to diverge, becoming higher in men. Chinese men had higher rates then Malay men whereas, the rates were higher or comparable between the women until the age group >70 when the rate for Chinese women overtook their Malay counterpart. Conclusions: Our study showed that there is a declining trend in the incidence of gastric cancer and higher rates were observed in men and Chinese.

RESUMENEl proceso adaptativo que conlleva al diagnóstico histológico de metaplasia intestinal es amplio y complejo; esta patología puede llevar a la aparición de cáncer gástrico, se ha sugerido a esta entidad como premaligna, y requiere una vigilancia endoscópica e histológica más estricta. Objetivos: Determinar la correlación entre los hallazgos endoscópicos sugestivos de metaplasia y su contraparte histológica en nuestro medio , dentro de ellos también determinar algunos factores asociados .a los pacientes con sospecha de metaplasia. Métodos: Estudio observacional analítico, realizado en el Hospital Nacional Hipólito Unanue donde se incluyeron pacientes >= 18 años con sospecha endoscópica de metaplasia intestinal. La evaluación estadística se realizó con el software estadístico SPSS. Resultados: 3398 pacientes sometidos a endoscopia alta se identificaron un total de 176 pacientes con sospecha endoscópica de metaplasia. En 152 de ellos se encontró confirmación histológica MI equivalente a un 86,36% del total de la muestra 51(29%) pacientes presentaron gastropatía atrófica en alguna parte de su estómago, 28(15,9%) pacientes presentaron gastritis superficial con la toma de biopsia. Predominaron pacientes de sexo femenino con MI (90). Los diferentes subgrupos generados por el reporte histológico de metaplasia intestinal incluyeron los siguientes: metaplasia completa (44,3%), metaplasia incompleta (17,6%), metaplasia inespecífica (13,6%), metaplasia mixta (11,4) % y la no presencia de metaplasia 13,1%. solo el 4 % de la población estudiada se reportó metaplasia intestinal con algún tipo de displasia . En el análisis bivariado se encontró asociación de riesgo con gastritis crónica atrófica OR: 3,733 (IC 95% 0,941-14,819, p = 0.051), gastritis crónica superficial OR 1,378 (IC 95% 0,382-4,973, p = 0.623) edad <=65 años con Helicobacter pylori (HP) y metaplasia intestinal + OR: 2,706 (IC 95% 0,342-21,402, p = 0.327)). Entre reflujo y MI, se encontró un OR 1,164 (IC 95% 1,096 -1,238, p = 0.322), HP y MI OR 0.537 (IC 95% 0.226 -1,277, p = 0.155) no se puede afirmar que existe una tendencia de asociación entre presencia HP y MI. Conclusión: Con respecto al diagnóstico histológico de metaplasia intestinal más de 2/3 de los pacientes fueron confirmados como positivos. Se encontró asociación de riesgo en los pacientes con gastritis atrófica, gastritis crónica superficial, edad<=65años y HP+, reflujo biliar, sin ser significativamente estadístico. Background Based on the results of several case-control and cohort studies gastrointestinal X-ray (GI X-ray) has been recommended for use in the nationwide screening program for gastric cancer.. Although this was the only effective screening program when almost all of the Japanese population were Helicobacter pylori (H. pylori) positive, there has been concern whether an alternative effective screening system should be established for the future H. pylori-negative generation. We therefore conducted the first randomized controlled trial (RCT) comparing GI X-ray and gastrointestinal endoscopy (GIE) scheduled according to results of serological testing (ST); this was done to determine the potential for an alternative screening method. Methods Subjects who fulfilled the inclusion criteria were residents between the ages of 30 and 74 and who were able to receive gastric cancer screening in the Yurihonjo area. Participants were assigned to the GI X-ray group or the GIE-ST group by computer randomization. Subjects in each group were further subdivided into 4 categories according to their different risks for gastric cancer. The feasibility of stratified randomization was serologically assessed and detection rates of gastric cancer at entry by the different screening methods were also compared. Results Of the 2,962 subjects invited, 1,206 individuals (41 percent) were included in the first stage of this stratified RCT, and 604 and 602 individuals were assigned to the GI X-ray group and the GIE-ST group, respectively. There were no statistically significant differences in sex, age, height, body weight, smoking, alcohol intake and family history of cancer between the 2 groups. During ST the GI X-ray group showed a distribution that was not statistically different from that of the GIE-ST group. Although 3 cases of gastric cancer were detected in the GIE-ST group, there was no statistically significant difference between the 2 groups. One complication found was barium aspiration during the examination in the X-ray group. Conclusion We confirmed that baseline demographic features of the 2 groups were well balanced.
