Ethnic Disparities in Ischemic Stroke, Intracerebral Hemorrhage, and Subarachnoid Hemorrhage Incidence in The Netherlands

Abstract: Background and Purpose-Data on the incidence of stroke subtypes among ethnic minority groups are limited. We assessed ethnic differences in the incidence of stroke subtypes in the Netherlands. Methods-A Dutch nationwide register-based cohort study (n=7 423 174) was conducted between 1998 and 2010. We studied the following stroke subtypes: ischemic stroke, intracerebral hemorrhage, and subarachnoid hemorrhage. Cox proportional hazard models were used to estimate incidence differences between first-generation et… Show more

“…The Dutch were chosen as a reference because they are the majority ethnic group in the Netherlands, and most non-Dutch minority groups are known to display a higher risk of CVD than the Dutch. 25,26 The results of logistic regressions are reported as odds ratios (ORs) with 95% CIs. In the regression analysis, we first used a univariate model with only family history as an independent variable (model 1), then adjusted the analysis for age and sex (model 2), and additionally adjusted for smoking status and educational level (model 3).…”

“…Second, we analyzed the relationship separately for each ethnic group by calculating the significance level of the interaction between positive family history of CVD and ethnicity, using Dutch as a reference group, and showing the ethnicity‐specific estimate of the association of family history of CVD with nonstroke CVD and stroke. The Dutch were chosen as a reference because they are the majority ethnic group in the Netherlands, and most non‐Dutch minority groups are known to display a higher risk of CVD than the Dutch . The results of logistic regressions are reported as odds ratios (ORs) with 95% CIs.…”

“…In the Loire Valley atrial fibrillation project, the relative risks of major bleeding also did not increase amongst elderly patients with ageing [ 21 ]. The disability and mortality associated with subtypes of stroke could also be related to ethnicity [ 22 , 23 ].…”

“… 16 This probably results from increased systolic pressure amplification in the setting of low central systolic BP. 17 Finally, while ethnic differences in CVD are well recognized, 18 20 data regarding differences in hypertensive phenotypes, including ISH, in young to middle-aged subjects from different ethnic groups are lacking. We previously identified substantial ethnic differences in BP and central haemodynamics.…”
