BACKGROUND The presence of pharmaceutical compounds has been studied in several countries in Latin America, and many xenobiotic compounds have been identified in wastewater, surface water and even drinking water, including ibuprofen and diclofenac. Microalgal‐based systems have been identified as a good alternative for removing these micropollutants, particularly under tropical conditions, and they are associated with low operational cost and high removal efficiency. Although studies for removing pharmaceutical compounds with Parachlorella kessleri are scarce, this microalga has demonstrated good performance in the treatment of wastewater. RESULTS Using doses of ibuprofen of 1000 and 16 000 μg L−1 and of diclofenac of 100 and 500 μg L−1, the removal efficiency of P. kessleri was evaluated under controlled conditions for periods of 12, 48 and 96 h during cell growth. A hormesis process was identified after an alarm phase at the first 12 h and a recovery at 48 h for ibuprofen, because the diclofenac curve achieved an early death phase, which was associated with oxidative damage. Total removal of up to 51.3% and 55.7% was obtained for ibuprofen and diclofenac, respectively. These removal efficiencies were mostly influenced by microalgae by the process of bioaccumulation/adsorption (23.3% for ibuprofen and 37.1% for diclofenac) and by other mechanisms (40.6% for ibuprofen and 44% for diclofenac). CONCLUSION Removal efficiencies were mainly associated with the presence of microalgae and removal mechanisms such as bioaccumulation and biodegradation for diclofenac, and biodegradation for ibuprofen. © 2021 Society of Chemical Industry (SCI).

Modernization is marked by an increase in pollution-related health risks due to the rapid economic and technological growth. In the Global South, there is concern regarding the disease burden attributable to chemical contaminants released into the environment, especially in river basins, where populations are exposed through multiple pathways. While significant research exists linking chemical exposure to disease, mostly in occupational settings, less data is available for the open environment. Emerging pollutants have shown ecotoxicological and genotoxic effects at low concentrations in both terrestrial and aquatic ecosystems, with humans at the apex. Despite such evidence, water quality standards focus primarily on communicable diseases risks, and the burden of disease method mainly includes gastrointestinal and respiratory infections. Therefore, the use of DALYs as a prioritization strategy needs to be evaluated to suggest alternatives for integrated analysis. Our aim was to enhance the comprehension of watersheds as socioecological entities that necessitate inclusive indicators to display the intricate and diverse links between water, health, and the resultant inequities. Based on previous research and information collected in a tropical Andean watershed at southwestern Colombia, we characterized drivers of pollution and health hazards. Using standardized methods for calculating DALYs, we estimated the water-related environmental burden of communicable and noncommunicable diseases. Estimates of disease burden were analyzed to find out disparities driven by ethnic, gender and socioeconomic status. Results indicate that significant inequalities persist affecting the most vulnerable populations for preventable communicable diseases. Furthermore, the impact of noncommunicable diseases, which are linked to chemical pollution from individual substances and their cocktail effect, remains largely invisible due to insufficient data and research. The econometric approach of the DALY addition effect needs to be expanded with historical and critical perspectives to expose the profound social and health inequities present in the socioecological systems of the Global South.

In addition to water-related communicable diseases, there are emerging concerns regarding the burden of disease attributable to the complexity of chemical pollution loads released into the environment. Studies demonstrating an association between chemical exposure and the occurrence of disease are abundant, particularly in occupational settings, although fewer assessments are available for the open environment. Agrochemicals, pharmaceutical compounds, disinfection byproducts, heavy metals, and many other emerging chemicals in very small concentrations, plus the mixture (cocktail effect) of several pollutants have shown ecotoxicological and genotoxic effects among various species in the trophic web of both terrestrial and aquatic ecosystems, at the apex of which are humans. Despite this evidence, water quality standards focus mostly on communicable diseases risks, and the widely promoted burden of disease approach mainly integrates the effects of gastrointestinal and respiratory infections. Based on previous research and information collected in a tropical Andean watershed at southwestern Colombia, we characterized drivers and hazards of disease and estimated water-related environmental burden of communicable diseases and an approximation to the likely burden of noncommunicable diseases. Estimates of disease burden are analyzed to find out disparities driven by ethnic, gender and socioeconomic status. Results show that profound inequalities persist affecting the most vulnerable populations for preventable communicable diseases. In addition, lack of information and more research continues to make the impacts of noncommunicable diseases, related to chemical pollution from individual substances and their cocktail effect alike, largely invisible. The DALY addition effect and its econometric approach ought to be enriched with historical and critical perspectives to make visible the profound social and health inequalities immersed in the socioecological systems of the global South.
