Methicillin-resistant Staphylococcus aureus (MRSA) colonization increases the risk of infection. Response to decolonization treatment is highly variable and determinants for successful decolonization or failure of eradication treatment are largely unknown. Insight into genetic predictors of eradication failure is potentially useful in clinical practice. The aim of this study was to explore genetic characteristics that are associated with MRSA decolonization failure. This cohort study was performed in a tertiary care hospital in the Netherlands. Patients with ≥ 1 positive MRSA culture from any site and with available whole -genome sequencing data of the MRSA isolate between 2017 and 2022 were included. Lineages, resistance, and virulence factors were stratified by MRSA decolonization outcome. In total, 56 patients were included: 12/56 (21%) with treatment failure and 44/56 (79%) with successful decolonization (with or without preceding treatment). A significant association was found between ciprofloxacin-resistant lineages and failure of eradication (OR 4.20, 95%CI 1.11–15.96, P = 0.04). Furthermore, livestock-associated MRSA and the major community-associated MRSA lineages ST6-t304 and ST8-t008 were associated with successful eradication treatment or spontaneous clearance. In conclusion, this explorative study showed a higher eradication failure rate in complicated MRSA carriers with ciprofloxacin-resistant MRSA lineages, which are predominantly healthcare-associated. Further studies are warranted to confirm the higher eradication failure risk of ciprofloxacin-resistant lineages, and identify the underlying mechanisms.

Staphylococcus aureuscauses both hospital and community acquired infections in humans worldwide. Due to the high incidence of infectionS. aureusis also one of the most sampled and sequenced pathogens today, providing an outstanding resource to understand variation at the bacterial subspecies level. We processed and downsampled 83,383 publicS. aureusIllumina whole genome shotgun sequences and 1,263 complete genomes to produce 7,954 representative substrains. Pairwise comparison of core gene Average Nucleotide Identity (ANI) revealed a natural boundary of 99.5% that could be used to define 145 distinct strains within the species. We found that intermediate frequency genes in the pangenome (present in 10-95% of genomes) could be divided into those closely linked to strain background ("strain-concentrated") and those highly variable within strains ("strain-diffuse"). Non-core genes had different patterns of chromosome location; notably, strain-diffuse associated with prophages, strain-concentrated with the vSaβ genome island and rare genes (<10% frequency) concentrated near the origin of replication. Antibiotic genes were enriched in the strain-diffuse class, while virulence genes were distributed between strain-diffuse, strain-concentrated, core and rare classes. This study shows how different patterns of gene movement help create strains as distinct subspecies entities and provide insight into the diverse histories of importantS. aureusfunctions.

Multiresistant bacterial strains are a very serious clinical problem in a hospital environment. Solving these types of nosocomial infections requires new and unique approaches. It is known that vancomycin is an antibiotic used for the treatment of serious infections such as S. aureus strains. Nanomedicine using new nanotechnologies brings new options like application of surface coating modifications for the targeting treatment. Superparamagnetic iron oxide nanoparticles (SPION) particles were created by a physicochemical method of synthesis. For the purpose of this testing, SPION particles have been modified by chitosan (300 rpm, 25 ℃, 24 h), then purified on the magnet. SPION/Chito particles were modified by vancomycin (300 rpm, 25 ℃, 6 h). The completed particles were washed with a PBS buffer (pH 7). The concentration of vancomycin (after the modifications) in SPION/Chito/VANCO particles was 100 µg/ml. The stability of finalized particles was tested in PBS (pH 7). The concentration of vancomycin was determined by HPLC. After that, the antibacterial activity was determined by measurement of growth curves (methicillin sensitive and resistant S. aureus) at 450 nm/540 nm for 24 hours (25 °C, 300 rpm in LB medium). The results were evaluated as AUC growth curves. Bacterial cultures were grown reproducibly in pure medium (AUCs were considered baseline).The study provided initial information about the effect of newly prepared types of nanoparticles on S. aureus species.
