ObjectivePeroneus Longus Tendon (PLT), a viable anterior cruciate ligament (ACL) graft, shares similar biomechanics, making it suitable for reconstruction. Controversy exists over whether PLT transplants affects the donor ankle joint. The purpose of this study was to examine the recovery of knee joint function following arthroscopic ACL restoration using autologous PLT and its influence on the donor ankle joint.MethodsA retrospective analysis was conducted on 65 patients with ACL rupture who underwent PLT graft reconstruction in our hospital from January 2016 to December 2021. A three‐dimensional gait analysis of the bilateral knee and ankle joints was performed postoperatively using an Opti_Knee three‐dimensional motion measurement and analysis system—Yidong Medical Infrared Motion Gait Analyzer. Knee function scores and changes in the range of motion of the bilateral knee and ankle joints were collected. The analysis of preoperative and postoperative joint function scores, bilateral knee and ankle mobility was performed by t‐tests.ResultsOne year after surgery, the patients' International Knee Documentation Committee (IKDC) scores, Knee Injury and Osteoarthritis Outcome Scores (KOOSs), and Lysholm scores were significantly improved compared to preoperative scores, with statistically significant differences (p < 0.05). There was no statistical difference in the American Orthopedic Foot and Ankle Society (AOFAS) score of the donor ankle joint before and after surgery (p > 0.05). During different gait cycles, there was no statistical difference in knee joint mobility between the affected and healthy sides (p > 0.05), but there was a statistical difference in the inversion and eversion angle of the donor ankle joint during the support phase (p < 0.05).ConclusionACL reconstruction using the PLT can yield satisfactory knee joint function. However, it does affect inversion and eversion in the donor ankle joint, necessitating postoperative exercises. Similar subjective function ratings for both operated and non‐operated feet, despite increased inversion‐eversion motion in the operated foot, may be influenced by the subjective nature and margin of error in the AOFAS Ankle‐hindfoot score, along with the relatively small variation in ankle inversion‐eversion angles.

PurposeThe association between the prevalence of patellofemoral arthritis (PFA) and femoral tunnel positioning following isolated medial patellofemoral ligament reconstruction (MPFLr) has not been well described. The aim of this study was to analyse the relationship between femoral tunnel positioning and the prevalence of PFA.MethodsThis was a single‐centre study of patients undergoing an isolated MPFLr between 2006 and 2011 with a minimum of 10 years of follow‐up. Outcomes assessed were the presence of PFA on radiographs, recurrence of instability requiring revision surgery and patient‐reported outcomes, including Kujala, Tegner and IKDC scores. Tunnel positioning was assessed on postoperative radiographs using two radiographic methods: Schöttle's point and the grid method to localise the femoral tunnel. Patients were grouped based on tunnel positioning and compared.ResultsFifty patients were analysed at a mean follow‐up of 12.4 years. Thirty‐three patients (66%) had a femoral tunnel position within 7 mm of Schöttle's point and 39 (78%) within the anatomic quadrant, with the most common location according to the grid method in D4 (28%) and E4 (26%), respectively. Thirty‐seven patients (74%) had a satisfactory (>80 versus <80) Kujala score at long‐term follow‐up. None of the examined tunnel assessment methods demonstrated a significant relationship with Tegner, Kujala or International Knee Documentation Committee scores. Patients with a femoral tunnel position >7 mm outside Schöttle's point or were considered to be in a nonanatomic position were not significantly more likely to result in unsatisfactory Kujala scores at the last follow‐up. Tunnel positioning and the other tested parameters were not found to be significantly associated with the development of PFA.ConclusionNo correlation between femoral tunnel position and risk of PFA or poor outcomes was observed in patients undergoing isolated MPFLr at long‐term follow‐up. The impact of femoral tunnel placement on long‐term outcomes in patients with PFI may be less significant than originally considered.Level of EvidenceLevel IV.

Purpose Residual anterior knee pain is one of the most common problems after total knee arthroplasty (TKA). However, the contributing factors afecting postoperative anterior knee pain (AKP) remain poorly understood. This study aimed to evaluate the efect of preoperative patellar bone marrow lesions (BMLs) and patellar cartilage defects on postoperative AKP after patellar non-resurfacing TKA. Methods This retrospective study included 336 patients who underwent unilateral TKA without patella resurfacing. All patients underwent preoperative magnetic resonance imaging (MRI) to assess the presence of BMLs and the degree of cartilage defects in the patella. Patients were categorized into four groups according to the presence of BMLs (with or without BMLs) and the degree of cartilage defects (with or without full thickness cartilage defects). The Kujala Anterior Knee Pain Scale (AKPS) and the Hospital for Special Surgery Knee Rating Scale (HSS) scores at 2 years after TKA were compared among the groups. Results Preoperative BMLs in the patella were found in 132 (39.3%) of 336 cases. Among the four groups, the group with both BMLs and full-thickness cartilage defects demonstrated signiicantly lower AKPS compared to the other groups at 2 years after TKA (p < 0.01), but no signiicant diference was shown in the HSS scores, between these groups. There were no signiicant diferences in either AKPS or HSS scores among the other three patient groups. Conclusions The presence of preoperative BMLs with full-thickness cartilage defects in the patella was associated with worse postoperative AKP after TKA without patella resurfacing. Patella resurfacing should be considered in this patient group to minimize the risk of developing residual AKP after TKA. Level of evidence III.
